We’re seeking social workers and mental health professionals who represent the communities we serve and are dedicated to removing barriers to adults who experience intersectionality to join our team as the
Mental Health Consultant - Intensive Care Coordinator
. The Intensive Care Coordinators (ICC) function within the Health Department’s Behavioral Health Division Care Coordination Services. As a member of this team you will provide Intensive Care Coordination for Oregon Health Plan eligible individuals with complex behavioral health needs including substance use disorder, mental health diagnoses, and co-occurring conditions. You will utilize your clinical knowledge of behavioral health conditions and in depth knowledge of the system of care and community resources to provide care coordination that is client centered, utilizes an integrated health approach, is trauma informed, culturally responsive, and linguistically appropriate. This position is part of the Adult Intensive Care Coordination (AICC) team and will primarily work with adults and depending on program capacity and needs may provide ICC support to youth and families. In this position you will perform a variety of mental health professional services including assessment, intervention, and direct clinical treatment services.

As the Mental Health Consultant - Intensive Care Coordinator - duties include:
Intensive Care Coordination
-
Provide telephonic and community-based care coordination to eligible members and their families.
-
Ensure treatment recommendations are understood by the members and providers.
-
Assist members through transitions between levels of care or different treatment providers.
-
Facilitate communication between members, their support systems, other community based partners and clinical care providers and ensure care plans are shared as needed.
-
Coordinate a multidisciplinary team to support complex member needs.
-
Support and assist with discharge and transition planning when appropriate.
Screening, Assessment and Care Planning
-
Conduct screenings of referred members to determine need and appropriateness for Intensive Care Coordination.
-
Review information included in referral, available in EVOLV, Phtech, Pre-manage, and other sources as needed.
-
Utilize standardized screening tools and clinical consultation to make determination.
-
Coordinate with regional CCO partners as needed to refer to lower level of care coordination.
-
Assess and identify strengths and needs through member contact, review of available clinical and utilization information, contact with providers, community partners and other members as appropriate.

Utilize assessment and member preference to create an individualized care plan for the member.
Documentation and Compliance
-
Maintain timely and accurate documentation about each member.

Provision of Care Coordination and Documentation of activities adheres to the Oregon Administrative Rules for Intensive Care Coordination.

Maintain tracking and data entry for program reporting and evaluation.

Utilize Hospital System Electronic Medical Record, CareOregon Electronic Care Management System and other systems as needed to gather information needed to provide comprehensive care coordination for members.

Education/Certification: Must meet the standards of Qualified Mental Health Professional - (QMHP - C) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) - more information can be found at

. QMHP - C and/or licensed Board of Oregon (LPC, LCSW, PHD,etc) (Required)

Graduate degree in psychology or

Graduate degree in social work or counseling or

Graduate degree in a behavioral science field

Understands , racial, cultural and ethnic factors, including systemic advantage or disadvantage; acculturation, intergenerational immigrant family challenges, effects of trauma during immigration, war, separation from family and/or primary supports, detention, refugee camps, etc.; generational trauma, and any factors related to non-dominant culture affecting mental health and treatment.

Ability to exercise good judgment, diplomacy, and tact in dealing with sensitive, complex, confidential information and situations, in addition to acting professionally, and empathically, with individuals, internal/external partners and communities.
