We are currently hiring for a Medical Records Clerk, at our Mesa Office!
Job Overview: Compile process and maintain medical records of hospital and clinic patients in a manner consistent with medical, administrative, ethical, legal and regulatory requirements of the healthcare system. Process, maintain, compile and report patient information for health requirements and standards.
SUMMARY OF FUNCTIONS:
Responsible for corresponding with patients when coming into the office or by phone, requesting their medical records, as well as other healthcare facilities.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Superior customer service skills which includes a friendly, positive attitude.
- Basic knowledge of the rules, regulations, policies and procedures related to patient medical records and patient confidentiality.
- Protect security of medical records to ensure patient confidentiality.
- Knowledge and ability to work with Outlook email, making sure you have your signature on the bottom of the email.
- Checking Outlook email to see if there are any requests needed asap.
- Check voicemails and personal desktops
- When chart prepping, make sure you check if there are any add-ons for that day.
- Retrieve patient medical records for physicians, technicians and other medical personnel.
- Review patient chart and ensure all necessary documents are obtained for continuity of care and are imported into chart prior to appointment.
- Import and attach testing results such as ultrasounds, EKG strips, Pacemaker strips, etc. to existing documents.
- Chart Prep 2 weeks in advance
- Answer phone calls in a professional manner
- Move over faxes from outside facilities and scans in office from the Outlook medical records inbox to the fax queue (Indexing Client). Labeling them correctly.
- Scan internal and external documents into electronic medical record and route document to appropriate desktop, if applicable.
- Fax testing to Primary Care Doctor or appropriate Healthcare facility once testing is finalized.
- Fill records requests and release information to persons and agencies according to regulations. (i.e., Patient, physicians, insurance companies and legal entities).
- Route records requests to our copying service Bactes(Sharecare), if requesting all medical records, or 5 years of medical records from Healthcare facility, other copying services(3rd party), Law offices, Insurance companies, Subpoena and Disability.
- Pull patient records from Banner and Dignity portals. Importing records into the patient’s chart.
- In the Doctor’s MA desktops make sure you go through and attach testing that is finalized in medstreaming into the patient chart and route to the medical records desktop to be faxed to primary care doctor or other healthcare facility.
- Maintains strictest confidentiality.
- Operate a Fax machine
- Other duties as assigned.
OTHER REQUIREMENTS:
- Ability to multi-task effectively.
- Must be able to act calmly and effectively in a busy or stressful situation.
- Ability to clearly communicate effectively in the English language in person, by phone and in writing.
- Hearing adequate to perform job duties in person and over the telephone.
- Vision acuity adequate to perform job duties, including reading information from printed sources and computer screens.
- Must adhere to all policies and procedures, including but not limited to standards for safety, attendance, punctuality and personal appearance.
- Must be able to establish and maintain effective working relationships with managers and peers.
- Exposure to Blood Bourne Pathogens (BBP) and Other Potentially Infectious Materials (OPIM) is rare but possible. Vaccinations and training are offered upon hire.
KNOWLEDGE, SKILLS, and ABILITIES:
Knowledge of principals and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, evaluation of customer satisfaction. Knowledge of administrative and clerical procedures and systems such as word processing, managing records, and other office procedures and terminology. Knowledge of the structure and content of the English language including the meaning and spelling of words, rules of composition, and grammar.
Ability to give full attention to what other people are saying and taking the time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Actively looking for ways to help people. Talking to others to disclose information effectively. Understanding written sentences and paragraphs in work related documents. Adjusting actions in relation to others’ actions.

EDUCATION:
High School Diploma or equivalent.
ALTERNATIVE TO MINIMUM QUALIFICATIONS:
Three to six months related experience and/or training; or equivalent combination of education and experience.
PHYSICAL REQUIREMENTS:
Ability to communicate clearly with patients, physicians, advanced practice providers, nurses, insurance carriers and in person. Hearing within normal ranges is helpful for normal conversations. Vision to perform job duties, including reading information from printed sources, computer screens and receiving common information and to prepare or inspect the documents.
Other requirements are occasional lifting and carrying items weighing up to 15 pounds unassisted. Requires some bending, reaching, standing, walking, squatting and sitting with some lifting, pushing and pulling exerted regularly throughout a regular work shift. . Repetitive hand movements (specifically keyboarding and writing). Assisting with patient mobility may be required. This may require you to on occasion wear gloves and / or other Personal Protective Equipment (PPE).
