Licensed Practical Nurse Supervisor Palliative
Job description
Concordia Visiting Nurses is seeking a LPN Palliative Clinical Supervisor for a full-time position which will report into our Administrative office in Cabot, PA.
The LPN Palliative Supervisor is responsible for the management and coordination of the palliative care services. The Supervisor ensures continuity, cost effectiveness and quality of operations. Facilitates communication and education, demonstrates leadership to those providing palliative care and acts as a liaison between Hospice and Concordia Visiting Nurses. Directly supervises all palliative care nurses, and members of the Interdisciplinary Team as assigned/appropriate. Oversees intake process to identify patients suitable for palliative care and facilitates appropriate interventions to manage symptoms and/or refers patients to hospice in a timely manner.

QUALIFICATIONS
- Licensed Practical Nurse with a current, valid Pennsylvania license.
- OASIS Certified Recommended
- Minimum of one [1] to two [2] years of hospice or palliative care nursing with roles reflective of progressive responsibility.
- Two [2] years of progressive experience in a home health care setting recommended.
- Interpersonal, communication and organizational skills.
- Proficient with knowledge of an electronic health record.
- Knowledge of agency policies, federal and state regulations and third-party payers
- Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a vehicle with insurance limits as established by the state
