The Intake Coordinator will collaborate with doctors, case managers, senior living communities including senior living community staff and CHC’s coding clinical supervisors to determine the focus of care and primary dx. This includes following up to discuss any issues, missing, or unclear clinical documentation and to schedule a doctor appointment or a visiting doctor.

General Responsibilities and Accountability
Referral Coordination • Ensure we can take the case regarding the insurance and requested care and add it to the referral log
- Based off of the requested care and diagnosis decide what disciplines will be needed
- Ensure all required documentation has been received; if missing anything call to request and document on the missing progress note list to keep track until received
- Coordinate with the marketers to ensure all details are clearly communicated
- Call the patient or their contact to explain our services and schedule the SOC
- Insure all dates, doctors and diagnoses are correctly filled in on our standard referral forms
- Hand off the referral to be staffed and input into WellSky; assist in getting the case staffed if any issues
- Once case is staffed, ensure it was done so correctly and fix if needed
- Ensure case was correctly put into WellSky regarding the patient’s profile, episode, disciplines assigned and any applicable contacts, insurance, orders and the visit pattern
- K-mail the referral as a PDF with a detailed message in WellSky to our internal staff, the disciplines assigned to the case and any applicable contacts
- Once referral is sent in WellSky, ensure it has been updated correctly on the referral log for the week and then hand off for the copies to be given to our internal staff
Initial Manage Care Authorizations:
- Obtain insurance plan and IO from referral source
- Run insurance information to determine eligibility
- Run through prompts / Call insurance company to gain authorizations
- Put authorization information into WellSky
Miscellaneous:
- Answer phone calls from patients, their contacts, doctors and as needed
- Manage the referral log
Education and Licensure
Required: High School Diploma
Required: Healthcare background.
Required: Computer skills including but not limited to MS Office Suite.
Preferred: Previous experience in an Intake Department.
Preferred: Previous experience working in an office environment and dealing with administrative tasks.
Preferred: Knowledge of Electronic Health Records and Electronic Practice Management systems preferred.
Competency and Skill Requirements
Excellent written and verbal communication skills. Able to utilize teams, email, and telephone for internal and external communications. Above average with Microsoft Office Suite (Work, Excel, Outlook, Teams)
Position Physical Demand
Sitting for extended periods of time, walking, standing while in office environments. Limited to lifting physical files/records and not expected to exceed ten (10) pounds in weight.
