Summary/Objective
The Homebase for Housing (HB) Specialist provides housing information and brokers service referrals to clients who are homeless or at imminent risk of homelessness in an effort to improve the quality of life of people living with HIV/AIDS in Dallas County and seven surrounding Counties.

Job Purpose: Assist clients who are homeless or at risk of being homeless in identifying affordable housing and links the clients to Ryan White and other support services.

Job Duties and Responsibilities:
1. Assist individuals who are homeless or at risk of homelessness move into or maintain stable and long-term housing
2. Conduct client interviews to screen for eligibility, identify barriers to self-sufficiency, coordinate access to mental health and medical providers and assess client needs
3. Screens & refers applicants to Legacy’s Master Leasing supportive housing program
4. Complete HB client intake forms at enrollment, maintain client files, perform updates and program exits
5. Develop individualized housing plans for clients enrolled in HB program
6. Document client enrollments, HUD data and client service referrals in the Homeless Management Information System (HMIS) database
7. Provide one-on-one client support services pursuant to housing to include but not limited to: conducting apartment searches, creating emails/user profiles to submit online public housing applications and to apply for medical, food and income related benefits
8. Participates in & supports in varying community and AIDS Services Organizations events concerning housing and homelessness in North Texas to include collaborative efforts of the Housing Forward’s Continuum of Care (COC)
9. Plan and facilitate 2 educational workshops each fiscal year for on the topics of Housing 101 and HIV 101
10. Performs community outreach to identify new and ongoing housing programs
11. Identifies and researches low-income leasing entities and community-based resources
12. Under the supervision of the Program Director, develops PowerPoint presentations for internal training and marketing purposes
13. Acts as data quality officer to ensure that housing information and resources on the program’s website are accurate
14. Contributes to team effort in performing related tasks and other duties as assigned

Required Education and Experience
1. Minimum of Bachelor’s Degree in a social science or equivalent work experience. If no degree at least 5 years of direct case management experience
2. Knowledge and experience in housing programs in North Texas required
3. Demonstrated ability to be a self-starter and to work independently
4. Demonstrated ability to work with diverse populations
5. Articulated passion for helping vulnerable populations and reducing homelessness

Job Type: Full-time

Pay: $40,000.00 - $45,000.00 per year
