Southern California Medical Center is looking for a Family Nurse Practitioner to join our mobile clinic team in Van Nuys.

Responsibilities, skills

• Primary Care Provider providing complete patient history, physical examinations, diagnosis, and plan for treatment of primary, secondary, and tertiary care services as outlined in, and in compliance with the standardized protocols for Nurse Practitioners. Primary care delivery includes communicating with patient or guardian of patient regarding care plans, results of diagnostic studies, and answering patient and family questions. Also included is organizing and delivering health education for patients, including general health information and disease specific education. The primary care provider coordinates patient referrals for specialized health care to public health care agencies.
• Establish and maintain a confidential medical record for each patient.
• Organize and administrate delivery of services offered by SCMC.
• Communicate with Chief Medical Officer/Assistant Chief Medical Officer regarding quality care and treatment issues of patients at SCMC.
• Conduct Clinical Pertinence Chart Review studies in order to assess and enhance patient outcomes.
• Write and revise policies and procedures for the mobile clinic and standardized protocols for Nurse Practitioners with the clinic administrator and the medical director.
• Develop, manage, and maintain statistical and informative data as well as other corporate documentation, including licensing and patient information.
• Coordinate clinic scheduling and site selection.
• Assure that services are provided in accordance with specified policies of SCMC and regulatory agency's policies.
• Serves as a resource for community agencies.
• Communicate with Executive Director, SCMC reporting on issues related to the functioning of the SCMC. Interact as appropriate with partner agencies of SCMC.
• Computer use for research, data entry, record keeping, and business communication via email. Administers medications or starts intravenous fluids, noting times and amounts on patients charts.

• Current licensure

Southern California Medical Center is a Federally Qualified Healthcare Center (FQHC). Nurse practitioners may be eligible for school loan repayment under the National Health Service Corps (NHSC) loan repayment programs in conjunction with the Health Resources Services Administration (HRSA).

Job Type: Full-time

Pay: $65.00 - $80.00 per hour
