Legacy Behavioral Health Services is hiring individuals with a passion for making a difference in their community! We are seeking a Discharge Planner to work in our BHCC in Valdosta, Georgia. The Discharge Planner under general supervision performs a wide range of tasks in the community to assist consumers in regaining control over their own lives while promoting the recovery process. Utilizing psychiatric rehabilitation interventions provides skills training, education and case management to consumers and their families. Monitor consumer progress towards treatment goals. Extensive documentation required; accurately charts services and consumer progress. Must be able to work well with community agencies and function as part of a team.

This position requires:
- Completion of a bachelor’s degree in social work, psychology, mental health, nursing, education, sociology, criminal justice, counseling, rehabilitation counseling, guidance counseling or an equivalent field AND one year of full-time experience in social services delivery.
- Knowledge of mental illness required
Working Conditions:
Must be physically and emotionally fit to work in a stress-filled environment to meet deadlines and deal with routine and crisis situations. May have extended periods of sitting, walking, and standing. Requires varying hours of duty to provide monitoring, educational and/or crisis handling, as well as travel to off campus locations in LBHS or personal vehicle.
