Overview:
The Chronic Pulmonary Disease Specialist provides respiratory expertise for patients with chronic lung disease, provides expertise in disease management, clinical pathways, and respiratory therapy protocols. They foster an inter-professional team that will cooperatively improve overall performance outcomes of specific diagnosis related groups (DRGs), such as length of stay and cost of care. They are also available for consultation to assist with patient education, to assist with discharge planning, and to ensure patients have the appropriate follow up after their discharge from the hospital.
Schedule : This position will be working 0.8 FTE (64 hours every two weeks). The shift length will be 8 hours on the day shift. This position will not work a weekend rotation.

As a Chronic Pulmonary Disease Specialist, you will be responsible for:
- Reviewing patient care according to clinical protocols to assist physicians and other health care team members in the development and implementation of standards of care.
- Collaborating in patient discharge planning process.
- Educating patients, families, respiratory therapists and other associates in disease management and clinical protocols for respiratory care.
- Collaborating with physicians and others team members to educate, establish goals, and implement the plan of respiratory care protocols.
- Performing chart reviews and audits to determine outcomes of clinical pathways and protocols.
- Participating with the Respiratory Care Director and Manager to review DRG financials and plans for improvement.
- Collecting health data from patient and family members to help in assessment of patient.
- Developing a plan of care in collaboration with patient, family, and other health care team members and implementing the services/needs as identified in the plan of care.
- Continually evaluating the patient’s progress toward the attainment of outcomes and then gathering, documenting, and relaying important information to the system.
- Interpreting spirometry results.
- Educating key stakeholders on Pulmonary Disease Management.
- Managing respiratory medication and providing patient education, outpatient education, and continuing patient follow up.
- Providing Smoking Cessation/Nicotine counseling, relapse prevention and education.
Required Qualifications:
- Education
- Bachelor’s Degree
- Experience
- 4 years of experience with documented competency in performing respiratory critical care duties and experience with chronic pulmonary disease patients.
- License/Certification/Registration
- Current License with Minnesota Board of Medical Practice.
- Registered Respiratory Therapist (RRT) issued by the National Board for Respiratory Care (NBRC).
- Basic Life Support (BLS) certification
