General Summary
The Day Rehabilitation Admission Coordinator will communicate with internal and external referral sources, Clinical Managers, and patients and families to develop the optimal patient schedule for admission to the program. Admission Coordination includes all aspects of patient pre-admission scheduling, including entering initial referral into EMR, communication of schedule and admission information to patient/family, referral source and clinical team, sharing basic insurance information as needed and navigating the DayRehab scheduling tool.

Principal Responsibilities
The Day Rehabilitation Admission Coordinator will:
- Receive and enter DayRehab referrals into the electronic medical record in a timely manner
- Confirm receipt of referral and other relevant information with referral source
- Communicate referral information to Clinical Manager/team
- Generate treatment schedule based on Clinical Manager recommendations and interpretation of current census of the clinic
- Enter schedule into DayRehab scheduling tool
- Confirm schedule with patient/family/referral source
- Assist clinic with rescheduling patient appointments as needed
- Perform all other duties that may be assigned in the best interest of the Shirley Ryan AbilityLab including assisting with administrative functions as needed.
Reporting Relationships
- Reports directly to the Director, Day Rehabilitation
Knowledge, Skills & Abilities Required
- Professional level of knowledge of rehabilitation.
- Prior knowledge of or experience working with electronic scheduling systems, and computer programs such as excel, word.
- Analytical skills necessary to plan for and make adjustments to complex treatment schedules.
- Strong attention to detail with ability to organize multiple aspects of clinic schedule.
- Interpersonal communication and advocacy skills to work with patients and families, and various medical professionals within and outside of the Institute.
- Excellent Customer Service capabilities.
Working Conditions
- Normal office environment with little or no exposure to dust or extreme temperature.
