Job description
· Provide assessment, training, and assistance to individuals with developmental disabilities enrolled in HCBS Waiver that need Site Based Day Habilitation services that include support on-site in the certified setting as well as in the community.
· To facilitate an individuals' independence, individualization, inclusion, and productivity.
· To facilitate on-site groups, instructional activities, and outings into the local community for inclusion and integration.
· Complete daily checklists, service documentation, and monthly summaries based on valued outcomes and goals from the Day Habilitation participant's Staff Action Plan.
· Complete weekly communication sheets to IRA's and family members requesting communication logs.
· Follow the community outings calendar and in-house group calendars for daily instruction. Facilitate ZOOM virtual Day Habilitation services when necessary and assist in developing & providing habilitative group activities.
· Ensure all case records and go and go files are up to date and complete. Assist with clerical tasks as assigned such as filing and supporting participants on the interoffice mail run.
· Administer medications, administer first aid, and help with toileting needs for individuals requiring support at the day habilitation site. (Must be willing to support diaper changes & wheelchair transfers)
· Support and advocate for individuals within the community. Provide linkages to community supports & volunteer services.
· Transport individuals to/from residences daily. Handicap van/wheelchair lift training required.
· All Day Habilitation Instructors will work collaboratively as a team to provide a productive, habilitative services.
· All Staff will maintain a caseload monthly; up to 8 individuals and be able to support the certified capacity of 44 individuals.
· All Staff must drive agency vehicles and adhere to agency policy & procedure for van and vehicle safety.
· All Staff must demonstrate flexibility and adaptability to work in a variety of setting within the community inclusive of volunteer sites.
· Perform all other duties as assigned.
- High School Diploma (minimum)
- OPWDD Fingerprint and Background Clearance
- Valid NYS Driver’s License and Insured Vehicle
- Clean driving record
Job Types: Full-time, Part-time
