Hennepin County Public Health (HCPH) is seeking a Community Health Program Supervisor to oversee the Intake, Referral, and Outreach team within the Family Health area of the department. This position will provide supervision, oversight, and leadership to the Intake, Referral, and Outreach staff who oversee the outreach and referral network for Family Home Visiting (FHV) and Family Health services in Hennepin County. This position will work with community, hospital systems, and providers to ensure quality service for families, will monitor data related to referral and intake activity, and will work with operated FHV teams and contracted providers to build and maintain a coordinated intake and referral system.

Location and hours:
This position is remote. Remote workers may be required to come on-site an average of 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours are Monday through Friday, between 8 a.m. to 4:30 p.m. with flexibility in hours. While this position is designated as remote, based on current requirements hires must reside in Minnesota or Wisconsin.

New employees who are hired into remote or hybrid positions between January 2, 2022, to December 31, 2024, will receive $500 toward the cost of establishing consistent internet connectivity, payable upon completion of 6 months of employment.

In this position, you will:
  • Oversee planning and implementation of FHV coordinated intake and referral system for operated FHV services and contracted providers implementing FHV models on behalf of the department.
  • Develop and implement outreach program strategies to streamline and coordinate activities across Family Health programs and services.
  • Monitor intake and referral data, identify trends, and address opportunities and barriers to outreach, referral, and intake.
  • Supervise, train, and evaluate Intake, Referral and Outreach staff, establish performance expectations, evaluate team and individual performance results, and provide coaching for development and improvement.
  • Represent the department on committees and task forces, act as a liaison with other county programs, state and local units of government, contracted providers, hospitals and clinics, and community stakeholders.
  • Prepare reports and other written materials for funders and HCPH leadership.

Need to have:

  • One of the following:
    • Six years or more of professional work experience in a related program area.
    • Bachelor's degree or higher in public health, public administration, community health, environmental health, nursing, behavioral/social science or other field appropriate to the position and two years or more of professional work experience in public health.
  • Access to reliable transportation to travel within the Twin Cites metro area.

    • Supervising and leading teams to develop goals and projects, implement work plans, and evaluate results.
    • Working with Family Home Visiting programs, data management for grant reporting and tracking FHV provider capacity, outreach and referral.
    • Working with clinical providers, building relationships with community stakeholders, working with multidisciplinary teams, and facilitating strategic discussions.
  • Ability to:
    • Gather and analyze data and information, design, test solutions to problems, and formulate plans.
    • Communicate effectively in both verbal and written form.
    • Demonstrate understanding of cultural differences and the impacts of racism on service provision, organizational effectiveness, and on relationships with staff, partners, and those we serve.
    • Use Microsoft or Google programs including Word/Google Docs, Excel/Google Sheets PowerPoint/Google Slides, and Outlook.
    • Be a strategic, adaptive, and creative thinker.
  • Knowledge of:
    • Public health approaches and practices.
    • The impacts of racism on health and the social determinants of health.
    • Project management and program planning.
