Avita Health System currently operates three hospitals and numerous clinic locations throughout Richland and Crawford counties. Over the last few years, the system has tripled in size serving multiple communities throughout the region. Clinical Pharmacist – Full Time (7 on / 7 off day shift)

JOB SUMMARY:

A clinical pharmacist is a decentralized health-system level pharmacist that works to provide optimal, safe, and evidence-based medication therapy to patients of all ages, backgrounds, and needs in a variety of settings throughout multiple health-system campuses. These clinicians provide bedside and consultative care services that include but are not limited to assessing patient needs, incorporating age and disease specific characteristics into drug therapy and patient education, performing point of care testing, adjusting care according to patient response, and providing clinical interventions to ensure quality patient outcomes relating to medication use. Clinical pharmacists also work directly with the Pharmacy Clinical Services Manager on a variety of topics such as precepting, training, education, guideline development, research, and quality initiatives.

Duties and Responsibilities

  • Ensures safe, appropriate, evidence-based, and cost-effective medication therapies for patients according to established policies, procedures, and protocols as well as patient age and needs.
  • Follows all pharmacy consults for assigned clinical areas and campuses. Rounds with medical staff and provides clinical consultation to practitioners as requested. Recommends evidence-based medication therapy regimens and monitoring plans. Suggests appropriate, cost-effective therapeutic alternatives to medical staff as needed.
  • Monitors medication therapy regimens to identify, prevent, or mitigate drug contraindications, drug–drug interactions, drug–food interactions, drug–laboratory test interactions, allergies, appropriateness of drug and dose, failure to receive drugs, untreated indications, use without indication, and therapeutic duplications. Takes appropriate action to resolve medication related problems to optimize care.
  • Provides direct patient care through activities such as admission/discharge medication review, reconciliation, and counselling.
  • Actively participates in stewardship activities and medication surveillance / restriction programs.
  • Identifies and reports suspected adverse drug reactions and medication errors accurately and in a timely manner.
  • Participates in medical emergencies (codes, STEMI, stroke, etc.) and serves as the medication expert in ensuring optimal medication therapies are timely and appropriate.
  • Documents clinical activities and interventions accurately, timely, and completely. Understands the utility in representing clinical tasks, interventions, and outcomes through data.
  • Facilitates opportunities for pharmacists to have positive impacts in patient transitions of care; identifies appropriate patients that have pharmaceutical care needs or risk factors that necessitate ongoing pharmacist intervention.
  • Participates in clinical research and practice advancement projects as well as patient care quality and regulatory compliance initiatives designed to improve medication-use processes or pharmacy practice.
  • Reviews, interprets, and validates medical literature to understand when research is applicable to clinical practice. Effectively communicates when new and emerging evidence should be considered for application within the organization; additionally, must be able to effectively identify and communicate research bias, confounding factors, clinical relevance, and when more research is needed.
  • Coordinates and/or conducts competencies to ensure clinical competency of staff pharmacists.
  • Identifies pharmacy staff knowledge gaps or teachable moments relating to medication use and effectively coaches and educates co-workers.
  • Fosters a team environment by providing orientation and training to new team members; assists co-workers in tasks as time permits.
  • Maintains proficiency in preceptor roles and actively emulates education/mentorship skills. Serves as a preceptor for assigned students, interns, and/or residents.
  • Leads the adoption of new and emerging clinical pharmacy and quality initiatives to improve patient care and/or advance the profession of pharmacy. Works to ensure colleagues are adequately educated and able to perform duties associated with initiatives.
  • Participates in all assigned departmental and inter-disciplinary workgroups, teams, and committees (antimicrobial stewardship, pharmacy and therapeutics, medication safety, patient care huddles, etc.).
  • Maintains clinical competencies as required for an inpatient staff pharmacist (e.g. ACLS, PALS).
  • Maintains active membership in state, national, or international professional pharmacy organizations.


Job Requirements:

  • Current Pharmacist licensure in good standing in the State of Ohio.
  • Graduated from an accredited School of Pharmacy.
  • Doctor of Pharmacy degree or Bachelor of Science degree or equivalent.
  • Three (3) years of relevant inpatient pharmacy experience as a pharmacist or completion of ASHP accredited PGY-1 Pharmacy Residency is required.
  • Knowledge of contemporary hospital/clinical practice and service required.
  • Significant knowledge and expertise in advanced pharmacy practice, clinical specialty medication therapy management, and evidence-based medicine in areas appropriate for clinical assignments.
  • Board of Pharmacy Specialties board certification in applicable clinical specialty is required; or must achieve board certification within 18 months of hire.
