Liberty Health Services is seeking to hire 2 Clinical Case managers to work at our brand-new center.

Liberty Health Services is a 24-Bed Medically Monitored detoxification facility located in Derry, NH. Our agency serves individuals suffering from substance use disorder in need of residential detoxification. Our proprietary detoxification protocols are designed to comfortably and safely eliminate physical dependence on substances in order to prepare client for the next appropriate level of care.

General Description

This position is primarily responsible for therapeutic and case-management services for clients receiving inpatient detoxification services. LADC preferred

Duties Include:

· Ensures compliance with DHHS and JACHO regulations

· Primary responsibilities to include clinical case management; milieu management and care coordination

· Work with interdisciplinary team to develop appropriate treatment and aftercare plans for each patient

· Completes comprehensive assessments, including provisional diagnoses and recommendations for services

· Therapeutic and Behavioral Crisis Intervention and de-escalation as necessary with clients

· Facilitates daily therapeutic groups

· Maintains consistent and professional communication with family, admissions and facility staff members

· Other duties as assigned

Job Type: Full-time

Pay: From $40,000.00 per year
