Description: This is an opportunity to represent The Center and follow up with discharged residential clients to assist them in carrying out their recovery plans.
Knowledge, skills and abilities:
· Ability to use lived experience to assist people in recovery.
· Ability to communicate effectively orally and in writing.
· Able to use Microsoft Office, electronic health records and other reporting programs.
· Willingness and ability to flex schedule as program needs change.
Primary Duties:
· Develop relationships with residential clients while they are in residence
· Become familiar with the discharge plan as well as the recovery plan for each client
· After discharge, communicate with the client and meet with the client periodically in order to monitor and encourage the client to stay in compliance with the recovery plan
· Represent CBHC and attend all meetings of the Metro East Recovery Council (MERC)
· Represent CBHC and attend all meetings of the St. Clair County Drug Prevention Alliance
· Represent CBHC as a liaison with Touchette Regional Hospital and Gateway Regional Medical Center and receive referrals for residential and outpatient services
· Provide referrals and coordination of referrals to appropriate agency services
· Maintain compliance with Joint Commission and other regulatory agency requirements
Reports to: Director Substance Use Services
Requirements:
· Lived experience with recovery
· Have or earn CPRS designation within one year of hire
· Maintain CPRS designation through continuing education
· High school diploma, GED, or higher education
· Valid driver’s license
· Vaccinated or willing to become vaccinated against COVID-19
Salary: $16.45 per hour without CPRS, $18.00 with CPRS
