Summerfield Healthcare is the preferred provider of Post-Acute Rehab services in Santa Rosa, CA and we are seeking a full time Occupational Therapist to join our team. This busy skilled nursing setting offers a stable in-house therapy department where there is ample opportunity for you to create and implement the programs that will best serve your patients. We have an active therapy intern program, experienced and cohesive therapy team, and excellent staff retention. We are a fast paced Post-Acute Rehab facility with a robust therapy program that caters to patients needing strengthening and reconditioning before transitioning home after an inpatient hospital stay. As a 56 bed facility our smaller size provides a more quaint and "home-like" family environment that allows for a more personalized approach to the delivery of patient care—an environment we feel plays a major role in the healing and speedy recovery of our patients. It also provides a unique opportunity for our staff to build a rapport with our patients and other staff alike that they otherwise would struggle to have in a larger facility. Recent renovations help us stand out amongst our competitors and we expect that you’ll notice right away when entering how clean and inviting our facility is.

THE CASELOAD

Summerfield has worked incredibly hard over the past couple of years to influence our reputation in the community and become the rehab facility of choice. This concentrated effort has enabled us to achieve a preferred partnerstatus with 3 of our local acute care hospital providers which has led to a steady increase in hospital-directed referrals. This has all but eliminated the ebbs and flows that most facilities experience with their rehab caseload and you can look forward to consistency with your hours here at Summerfield.

In addition, we have been awarded the prestigious Ensign Flag Award and are determined to win it again this year.
