POSITION: Case Manager

SALARY: $20.62 - $24.50, per hour

REPORTS TO: Clinical Director

LOCATION: Nespelem, WA

Basic Functions:

This position is responsible for supporting the coordination and engagement of care with current and prospective clients of HHS/BHP and help maintain and establish collaboration with other supportive programs, agencies and professionals.

Qualifications

MINIMUM QUALIFICATIONS:

Education and Training:

Typically requires and Associates' Degree plus five (5) years behavioral health related intake, case management, discharge planning, or related experience. Bachelor's degree in behavioral health, social services, or related discipline minimum of one (1) year behavioral health related experience (preferred) and may substitute for experience. A RN licensure may substitute for a bachelors degree.

Knowledge, Skills, and Abilities:

  • Requires knowledge of the responsibilities and resources of federal, state, tribal and local agencies responsible for administering chemical dependency, behavioral health and tribal health services.
  • Requires knowledge of and sensitivity to American Indian culture.
  • Requires substantial familiarity with case management principles, record keeping, and documentation.
  • Requires demonstrated skills in motivational interviewing techniques, objective analysis, problem-solving, organization, information gathering, time management, and documentation activities.
  • Requires critical thinking: Case Managers have to be able to understand what a person needs when coming to them in crisis or in a less than optimum state of health. Being able to find solutions for a myriad of issues will be necessary.
  • Requires problem solving: Case Managers have to find solutions for the problems their clients bring to them. Case Managers need to be able to make decisions on continuum of care as it relates to treatment.
  • Requires attention to detail: Case Managers review all of the patients medical history with a close eye so as not to miss any details that will interfere with recovery or care.
  • Requires compassion: Case Managers see people during moments of trauma or crisis, where they and their family are vulnerable or in pain. Being patient with people who may have trouble communicating or understanding what is happening and what is important.
  • Requires communication skills: Case Managers need to be good listeners and speakers. Case Managers communicate with patients and their families often and are the liaison between families and providers, including CD/BH therapists or other medical staff.
  • Requires organization: Case Managers often see multiple patients every day, with different needs.
  • Requires organization; keeping their records organized is crucial.
  • Requires strong written, communications, organizational, and interpersonal skills.
  • Requires experience working both independently and in a team oriented collaborative environment.
  • Requires knowledge of research methods, statistics, and data entry.
  • Requires knowledge of HIPAA compliance requirements.
  • Requires the ability to make critical decisions, based on integration of information from multiple sources in high-stress situations.
  • Requires the ability to identify problems, develop and implement solutions.
  • Requires the ability to maintain effective working relationships with the public, coworkers, and various Tribal, State, and Federal Programs.
  • Requires the ability to write and speak clearly, concisely and accurately.
  • Requires the ability to communicate effectively in writing to prepare reports, case summaries and correspondence in and understandable, clear and concise manner.
  • Requires the ability to maintain client records on paper and in the electronic health record.
  • Requires the ability to work under stressful situations to handle conflicts, complaints, and other problems in a professional and courteous manner.
  • Requires the ability to adhere to professional and ethical codes of conduct.
  • Requires the ability to safeguard confidential information.
  • Requires the ability to establish workload priorities to manage caseloads, work assignments and/or projects in an efficient, organizes and timely manner, to meet demanding deadlines while maintaining a professional demeanor.
  • Requires the ability to work independently with good judgement with minimum of supervision.
  • Requires the ability to be a team player.
  • Requires sensitivity to HIV/Aids, substance use, mental illness and other related issues.
  • Requires the ability to operate a computer and work with various computer software applications.
  • Requires the ability to safely operate a motor vehicle.
  • Requires strong family and individual assessment skills.
  • Requires the ability to provide service in culturally competent manner.
  • Requires the ability to create agendas, minutes, correspondence, reports and presentations.

Note: Pursuant to Tribal Policy, if this position is safety sensitive it is subject to pre-employment drug testing. In addition, this position is subject to reasonable suspicion and post-accident drug testing.

TRIBAL MEMBER AND INDIAN PREFERENCE WILL APPLY; PREFERENCE WILL ALSO BE GIVEN TO HONORABLY DISCHARGED VETERANS WHO ARE MINIMALLY QUALIFIED.

If required of this position, you must possess and maintain a valid Washington State driver’s license and be eligible for the Tribes’ Vehicle Insurance. In addition, this position may be subject to pre-employment background clearances. If applicable, these clearances must be maintained throughout employment.
