JOB DESCRIPTION

POSITION: Case Manager

LOCATION: Red Cliff Community Health Center

SUPERVISOR: Community Health Manager

SALARY: $18.00-21.00/ hour

THIS IS A REGULAR FULL-TIME NON-EXEMPT POSITION

JOB SUMMARY: This position is responsible for the coordination of appropriate, timely, and beneficial care. Utilizes established standards for case management to prevent disease or injury and promote quality health care outcomes. The priority is to promote teamwork within the microsystem to improve patient access, healthcare outcomes, and Covid-19 community recovery.

DUTIES AND RESPONSIBILITIES:

  • Responsible for active management of patients.
  • Collaborates proactively with all interdisciplinary team members with a patient care focus to facilitate and maximize client healthcare outcomes.
  • Identification of population-based care needs and provision of care.
  • Utilizes iCare clinical system to track, monitor and assure the appropriate follow-up of clients. Utilizes software packages and applications for ordering, scheduling and tracking patient care to ensure complete visits that max-pack for thorough care.
  • Assists with making meaningful improvement to services, programs, and processes and/or organizational effectiveness.
  • Knowledge of quality improvement models and process.
  • Completes PDSA cycles of improvement when changes/improvements occur.
  • Complete monthly, quarterly, and annual reports as required by Health Center Administration, and regulatory bodies. Maintain a record of all supporting documentation as required.
  • Adheres to and supports facility policies, programs and activities.
  • Maintain strict confidentiality and safeguard the privacy of patients in common areas of the clinic.
  • Present a professional, caring image for the Health Center and its programs.
  • Maintain a cooperative relationship with other Health Center staff and employees.
  • Demonstrate tact, courtesy, and respect in communication and interaction with Health Center patients, visitors, and staff and with outside agencies and programs.
  • Promote a working environment noted for effective cooperation and collaboration between programs, services, and co-workers.
  • Dress appropriately to promote professionalism within the Health Center.
  • Advance job knowledge and skills through continuing education efforts after approval of Health Center Administration.
  • Responsible for communication within the department and across departments to ensure that information is shared for effective operations.
  • Attend staff and other meetings, in-services, and other events as directed by supervisor.
  • Responsible for maintaining a clean and safe environment.

KNOWLEDGE:

QUALIFICATIONS:

  • Minimum of high school diploma, HSED or GED required.
  • Associate degree in Human Services or related field preferred
  • One year experience connecting community members to resources, preferred.
  • One year experience working with tribal communities, preferred.
  • Holds current Healthcare Provider Basic Life Support certification or willing to seek certification within 90 days of hire.
  • Must have a valid driver’s license, vehicle and appropriate vehicle coverage and maintain driver’s eligibility as a condition of employment. Be eligible to be put on the tribe’s vehicle insurance policy.

PERSONAL CONTACTS: Daily contact with clients, visitors, vendors, Tribal and Health Center Administration, and other Tribal program staff.

PHYSICAL REQUIREMENTS: The duties assigned to this position involve bending, stooping, lifting, and carrying. Items may be placed on overhead storage. Weights to be carried are usually less than 50 pounds.

WORK ENVIRONMENT: Red Cliff Community Health Center; office and clinic settings.

Exposure to hazards of the health care industry. Work setting must be maintained as a clean, non-smoking, well-ventilated area in compliance with all applicable safety regulations. All Tribal Buildings are smoke free.

TRAVEL REQUIREMENTS: Will be required to attend training and meetings both in state and out of state. May be asked to perform visits to patient homes.
