JOB TITLE: Community Psychiatric Supportive Treatment (CPST)
REPORTS TO: Clinical Director
WAGE: Negotiable
HOURS: Forty hours per week to accommodate clients and to address emergencies
Provides an array of services in the community to clients with severe and persistent mental illness. It is necessary to be mobile, work with a multidisciplinary team to assist the client in meeting recovery goals as outlined in an ISP. As a CPST, one should be able to assist clients with services that are specific and measurable to each individual person’s ability and needs. It’s primary focus is to provide services which will incorporate the use of evidenced-based practice in treatment toward attainment of goals in the ISP including the use of Best Emerging Practices. Service delivery may be face to face or on the telephone. Services may be provided individually or in groups. Services are not site specific but must be in locations that meet the needs of the persons served.
Domain I: Supportive Treatment
- Assist in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian.
- Partner with client to assess client’s needs incorporating outcomes in treatment planning and the development of recovery goals.
- Facilitate development of daily living skills as identified by the individual and/or parent or guardian.
- Provide symptom monitoring with clients to identify triggers, coping skill development, crisis management and relapse prevention.
- Meet at least monthly with each client or as stated in treatment plan.
- Ensure continuity of care for each client on designated caseload through assessment, service planning and outcome results.
- Understand and implement activities that increase the individual’s capacity to positively impact their own environment while partnering with the client.
- Understand and implement mental health interventions that address symptoms, behaviors, thought process, etc. that assist an individual in eliminating barriers to seeking or maintain education and employment.
- Ability to communicate verbally with a client
- Understanding and use of evidenced-based practices.
- Ability to identify goals for treatment in conjunction with the client and use of tools including the outcome survey to assist client in his/her recovery.
- Assist with education and training when appropriate to the family and/or guardian.

Domain II: Paperwork
- Completes notes and turn in SALS by 5pm the next business day after providing services
- Any needed releases of information are completed and updated.
- Requests for paperwork by client, SSA, DJFS, etc are filled out in a timely manner and returned to HI Dept.
- Record all activities and complete other written and statistical reports as indicated by agency/system procedures and client need such that keeps charts up to date in a timely manner for certification process.
- Coordination of the ISP: services are identified in the ISP; assistance with accessing natural support systems in the community; linkage to formal community services/systems; incorporate outcome survey results to develop new goals; monitor progress of client achieving his/her goals and be able to adapt goals as needed.
Domain III: Consultation
- Participate in multi-disciplinary staffing and risk management staffings.
- Participate in trainings and provide trainings as requested.
- Participate in development of programs to enhance knowledge, skills and personal service network.
- Willingness to contribute to solutions and resolution of problem areas.
Domain IV: Coordination
- Coordinate crisis management and stabilization as needed in the least restrictive environment.
- Able to coordinate psychiatric hospitalizations as indicated, both voluntary and involuntary.
- Provide advocacy and outreach to community resources to improve the quality of the client’s life.
- Maintain a cooperative relationship with employees, medical staff, community members and others.

Domain VII: Job Activity Demands:
Physical:
- Ability to lift and carry twenty five pounds of weight,
- Ability to sit, walk or stand for extended periods of time.
- Ability to scoop, squat, bend and have full use of range of motion.
- Must be able to drive a vehicle and transport clients in own vehicle
- Must be able to manage stairs.
- Must be free from health conditions which would restrict employee from entering a client’s home and/or apartment.
Mental and Personal:
- Have a valid driver’s license
- Have no more than 4 points on his/her license
- Must have proof of liability insurance
- The ability to travel through the community to make home visits.
- Ability to deal with conflict and to cope with potentially difficult and hostile clients.
- Sound judgment and ability to respond to unusual circumstances.
- Ability to relate to others under stress
- Basic understanding of mental health/mental illness concepts and symptoms.
- Understanding of basic human needs
- Knowledge of effects of long-term mental disability and possible substance abuse.
Education:
- The person must have a Bachelor’s Degree in a human service field or equivalent years of experience and education. Other individuals able to provide CPST services include: Medical Doctors, Licensed Practical Nurse; Registered Nurse; MSN; Independently Licensed Social Workers and Counselors; Social Work Assistance; Intern/Fellow and trained others.
