Title of Position:

Bilingual Wrap-Around Services Coordinator (Social Worker License Required)

Organizational Relationship or Line of Authority:

Works under the supervision of the Assistant Superintendent of Student Services and Equity and Supervisor of Health Services.

Salary:

Annualized salary of $75,000

This position is a partly grant funded position through the Comprehensive School Health Service Grant

Schedule:

Full-time, 12-month position, 218 days

8 hours per day, Monday through Friday (8am-4pm) with flexibility to shift schedule as necessary for evening work with parents

Location:

Registration and Parent Outreach Center, Marlborough Public Schools

The Comprehensive School Health Services Grant has for following purpose/requirements:

• Provide a case management program in school setting that addresses the needs of students with chronic health conditions so that they may attend school safely.
• Provide services that are community-based and culturally and linguistically relevant meaning that they address racial inequities and health disparities as appropriate.
• Assist families to obtain access to sustainable long-term access to health care.
• Grant Targets:
  • Decrease in chronic absenteeism
  • Decrease in students who are overweight
  • Increase in # of English Learner (EL) & McKinney-Vento (MKV) students seen by school nurse
  • Increase in mental health services for students
  • Increase in individual health care plans for students with special healthcare needs
  • Increase in referrals for students who fail screenings
  • Increase in # mental health assessments for students reporting bullying
  • Increase in # students reporting having at least one trusted adult at school

Statement of Duties:

• Serve as the Marlborough Public Schools (MPS) point of contact/liaison with outside agencies & organizations for supports, wrap-around services for students, including but not limited to, counseling, food bank, South Middlesex Opportunity Council (SMOC), Marlborough Hospital)
• Support enrolled students & families, including those who are newly enrolling in the district
• Primary resource contact for MKV and other families that demonstrate a need to provide support with food, shelter and health care resources
• Establish a database of existing resources within the MPS community, and create, update and maintain a working resource (document) for staff to readily access with wrap-around services available to students and families.
• Serve as a facilitator to school administrators and School Adjustment Counselors (SACs) on availability of these wrap around services.
• In collaboration with RPOC nurse and translators, assist families that require support with:
  • Completion & submission of MassHealth applications
  • Obtaining primary care physician, if applicable
  • Obtaining necessary immunizations & physicals
  • Applications for SNAP, Transitional Housing Assistance, etc.
  • Referrals to MetroWest Legal
• Consult with SACs regarding complex student & family issues-facilitates referrals to wrap-around services, as needed

Qualifications:

Bachelor’s Degree

Licensed Social Worker in the State of Massachusetts

Bilingual (Portuguese/English or Spanish/English) required

Must be able to work well independently as well as part of a team coordinating with internal staff and external agencies

Good time management skills and ability to juggle multiple priorities
