REQUIREMENTS: 1) License as Registered Nurse in good standing with the Texas Board of Nursing 2) Cardiac and/or Community Health Nursing experience is preferred 3) Critical Care nursing experience 4) Experience with post-acute care practices 5) Minimum 3 years’ experience in acute care setting is preferred 6) CPR certification 7) ACLS
8) RN Specialty Certification preferred.

Must have current licensure/certifications upon hire. New Hires will have 60 days to acquire the required Basic Life Support (CPR and ACLS) and/or any other life support certifications required for the job.

SUMMARY OF RESPONSIBILITIES: The 360 Care-Case Management Registered Nurse (RN) coordinates transitional and chronic care case management duties focusing on preventing hospital readmissions, decreasing unnecessary admissions, and improving patient outcomes. The RN assists in care coordination efforts and collaborates care with members of the inpatient interdisciplinary team to provide education, support, and follow-up for these patients and families in the inpatient and outpatient settings with the goal of improving overall health and wellbeing of the patient and ultimately reduce hospital readmissions. Knowledge and experience in adult education principles and/or patient care practice, care coordination, and clinical health coaching skills are necessary. The RN will actively participate in population health projects such as DSRIP and Medicare Shared Savings Programs, and leads efforts to reduce preventable readmissions through the Care Collaboration Group at WHS. Knowledge and experience in treating patients with Congestive Heart Failure, Chronic Obstructive Pulmonary Disease, Coronary Artery Bypass Graft surgery, Pneumonia, Sepsis, general skilled nursing facility/post-acute facility, and Acute Myocardial Infarction are necessary. RN must be willing to work on both inpatient and outpatient services. RN is expected to assist with nursing orientation, nursing education, community health fairs, and community resource management which may require some after-hours and weekend work. RN will conduct all outpatient tele management follow-up phone calls for patients enrolled in the program.
Must perform all assigned duties such as: Prepares data entry into data bases and EMR, provides transition coordination from the hospital setting by communicating with physician, interdisciplinary team, patient, and family. Participates in patient care conferences and monitors discharge preparations from admission through discharge on all patients on main campus. Facilitates transition process and patient understanding of instructions. Other duties include scheduling follow-up appointments as necessary and communicating with patient’s family and physician regarding transition process. Serves as patient advocate and facilitator throughout the transition process. Proficient with Microsoft Excel
Collects and documents Performance Improvement data regarding transition process, including readmissions, and participates on related committees. Maintains positive working relationships with staff and other departments to provide optimal quality outcomes within the framework of professional nursing practice. Establishes helpful and friendly relationship with patients, staff members, providers, manager/director, and other job duties as assigned by supervising provider, manager/director

REPORTING RELATIONSHIP: Reports directly to the 360 Care Management Director; Administratively to the CNO and CMO.

KNOWLEDGE AND ABILTIY: Exemplifies sound clinical judgment and experience and expertise in area of clinical management. Demonstrates organizational skills necessary to provide smooth transition from hospital environment through discharge process. Able to provide education at the patient/family level of understanding. Demonstrates acceptable computer skills.

OSHA TASK CATEGORY TWO: There is an occasional risk of exposure to mucous membranes or skin contact with blood, body fluids, or tissues, or a potential for spills or
splashed during the performance of the job. Universal precautions must be observed, and protective devices worn by the employee when performing a task that is likely to cause exposure.

PHYSICAL REQUIREMENTS: Able to lift 30 pounds on a regular basis to 42”, able to lift 10-20 pounds daily, able to lift 50-80 occasionally
