  • Weeknight On-Call LPNs

Summary of the Weeknight On-Call Licensed Practical Nurse (LPN) Position:

  • Position Summary: Would provide on-call service and after-hour care to current hospice patients. Responsible for assessing the patient/family/caregiver needs, planning for the care, implementing the Plan of Care, and evaluating the effectiveness of the care provided.
  • We have two open Weeknight On-Call LPN Positions based out of our Belleville location:

1.) North Territory Position would provide care to patients in following counties: Madison, Clinton, Bond, Montgomery, Macoupin, Jersey, and Calhoun.

2.) South Territory Position would provide care to patients in following counties: St. Clair, Monroe, Washington, and Randolph.

  • After hours, all calls go to our answering service and then the answering service calls the on-call RN and gives them the messages. You could take calls from your home. Some on-call situations can be handled over the phone, while others would require a visit.
  • Full-Time Non-Exempt Position
  • While it is an on-call position and the hours of actual time worked would vary, this position is guaranteed pay of 35 Hours weekly. (70 hours bi-weekly).
  • Schedule: Would be on-call weekdays evenings beginning at 5:00 P.M. until 8:30 A.M. the following morning. Would begin Sundays at 5:00 P.M. and end Fridays at 8:30 A.M.
  • If you happen to be actively on-call (pt visits or phones calls) and work more than 35 hours in a week, you will be paid the number of hours that you actually worked (for example if you work 40 hours in a week, than you would be paid at 40 hours instead of guaranteed 35. If you work less than 35 hours a week, you are still paid for 35 hours)
  • Paid additional pager time of $2.00 an hour for 155 hours bi-weekly since the RN would be on- call for that time frame.
  • Would also participate in a rotating on-call shift for holidays.

Position Requirements

Education

1. Graduate of State-approved school of nursing (LPN)
2. Successful completion of Licensed Practice Nurse education.

Experience
1. One (1) year experience in hospice preferred.

Other Qualifications
1. Current Illinois nursing license (LPN)
2. Certification in hospice and palliative care preferred.
3. CPR certification preferred.
4. Excellent leadership, managerial ability, clinical assessment, organizational and interpersonal relationship skills.
5. A working knowledge of Medicare/Medicaid, hospice regulations, and third-party payor expectations.
6. We visit our patients wherever they may located, whether it be in their homes, long-term care facilities, nursing homes, hospitals, etc. As a result, driving and reliable means of transportation are requirements of the position.

Essential Functions

1. Collaborates in the development and modification of the Field Staff Nurse/Case Manager comprehensive nursing plan of care for the hospice patient.

2. Performs physical assessment and collects data then collaborates with the Field Staff Nurse/Case Manager in the assessment of the health status of a patient.

3. Implements aspects of the plan of care, as delegated.

4. Participates in health teaching and counseling to promote, attain, and maintain the optimum health level of patients, as delegated.

5. Serves as an advocate for the patient/family/caregiver by communicating and collaborating with the IDT, as delegated.

6. Participates in the evaluation of patient responses to interventions. Provides direct patient care.

7. Participates in on-call after hours care.

8. Demonstrates basic LPN clinical skills. Gives a practical exhibition and explanation at the required annual skills day.

9. Knowledgeable in patterns of disease progression at the end-of-life and provides ongoing patient/family/caregiver education, as delegated.

10. Practices within the guidelines of the Illinois Department of Professional Regulation.

11. Provides evidence-based physical, emotional, psychosocial, and spiritual care under the direction of a Registered Nurse.

12. Demonstrates knowledge of the principles of end-of-life pain and symptom management.

13. Participates in monitoring symptoms and changes in patients and reports these changes to the Field Staff Nurse/Case Manager in charge of patient’s care.

14. Participates in on-call after-hour care. In the on-call role, the LPN triages calls and provides care under the direction of a Registered Nurse.

15. Participates in IDT meeting by providing information regarding patient’s current health state and observations pertinent to the plan of care.

16. Demonstrates the ability to work cooperatively with others in a team environment to ensure deadlines, goals, and objectives are met.

17. Demonstrates the ability to embrace change, manage stress, and maintain a positive attitude.

18. Builds relationships and collaborates effectively with internal and external customers while cultivating referrals and referral sources.

19. Organized and works efficiently with minimal supervision. Is a self-starter who demonstrates flexibility, motivation, initiative, and commitment. Ensures required assigned education is finished in the specified time.

20. Identifies and contributes to the development of new ideas and approaches to improve the work process.

21. Participates in QAPI activities as needed.

22. Exemplifies Hospice of Southern Illinois core values in daily practice.

23. Appropriately manages Paid Time Off.

24. Timely and accurate timesheet documentation according to Policy and Standards of Performance.

25. Other duties as assigned.

Physical Requirements: Performs repetitive tasks. Physical tasks including bathing, transferring, lifting, and turning of patients in the home or home-like setting. Prolonged periods of sitting at times. Exerts up to 100 lbs. force occasionally, and/or up to 50 lbs. frequently and/or 20 lbs constantly. Requires physical flexibility of bending at knee and squat-bend. The Staff Licensed Practice Nurse manages stress appropriately, makes decisions under pressure, handles multiple priorities, works alone, and manages anger/fear/hostility/violence of other appropriately. This position requires clarity of vision >20 inches and <20 feet, hears alarms/telephone/tape recorder.
