RN or LSW or Bachelor's degree in related field with three years paid clinical experience working with individuals with Behavioral Health diagnoses required.

Only those candidates in OH, or WV will be close enough to be considered for this position. This position is a remote position after training. Client assessments are done traveling from home and back so the applicant home location must be within reasonable proximity to the southern Ohio clients.

Provide initial assessment services to applicants with severe and persistent mental illness (SPMI) or diseases of chronic condition (DCC) that have been referred for Specialized Recovery Services Program.

Perform program eligibility assessments and propose initial service plans for applicants applying for the program.

Service plans include both services provided through the SRSP and referral and coordination of other state plan Medicaid and non-Medicaid community based services.

Establish relationships with community partners and service organizations involved in applicant’s care.

Work collaboratively with Behavioral Health or Medical Provider network and SRSP recovery managers to assist applicants with coordination of Behavioral Health or Medical services.

Enroll eligible applicants into SRSP and facilitate transition to Recovery Management services.

Pay: $44,000.00 per year
