Park Ave Extended Care is looking for a Full Time Spanish Speaking Social Worker. We are a 240 bed skilled nursing facility located in Long Beach. Preferablly the social worker would have expierence with long term care for at least one year.. However we will accept recent grads. Social Worker must have organizational skills, able to work in a team, dynamic people person customer oriented skills with experience in doing psychosocial assessment, MDS, comprehensive patient-centered care planning and skilled at discharge planning. The social worker must speak spanish as he/she will be working on a unit with spanish speaking residents who do not speak English. This is a department of 3 staff including a Director who all work very well together as a team. Social Worker reports to Director of Social Work.The social worker will be resposnible for our unit that holds 40 residents half of which are spanish speaking.
Will accept BA or MSW .
Social Worker Hours: Mon- Fri 9:00am- 5:00pm
