Qualifications
- A degree in Social Work is required. CSW or LSW required.
- Preferred at least three years of experience in long-term care.
- Verbal and written communication skills
- Excel Proficient
- Experience in completion of MDS
- Strong organizational skills as evidenced by the ability to multi-task
- Proven ability to work in a team environment.
- Demonstrated investigation skills related to customer complaints and other resident concerns.
- Working knowledge of Microsoft Word,
- Able to lead others and organize weekly Interdisciplinary team meetings.
- Strong documentation skills
