Bear Mountain Healthcare is looking for an experienced, motivated, and creative Social Worker for our MA locations. We have locations in the Boston area, Springfield area, Worcester area, North Shore & South Shore areas. The Social Worker is responsible for meeting the psychosocial needs of residents. Additionally, the SW shares a responsibility toward creating and sustaining an environment that humanizes and individualizes each resident. Specific tasks include but are not limited to:

Ensuring state and federal regulatory compliance is met

Utilizing community resources that are useful to residents and their families/significant others

Managing legal documentation regarding guardianships and healthcare proxies
-
Working with the interdisciplinary team and administration to promote and protect resident rights and the psychological wellbeing of each resident
-
Preventing and addressing resident abuse as mandated by law and professional licensure
-
Planning safe discharges for residents
-
Completing MDS Assessments
-
Ensuring appropriate documentation is in place
