Social Worker (MSW) Full-Time
Job description
The MSW investigates, assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Assists with discharge planning and processs. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
Takes Lead role in social work support for psychiatric patient population.
Performs Medicaid screenings as indicated for NHP.
Assists patients with Safety Net applications when needed.
Full-Time, 40 hours per week. Master's Degree in Social Work required. State of TN Licensed Master Social Work required. Three years of clinical experience as a Social Worker required and Psychiatric Social Work experience required.
