Job description
Duties:
- Direct care plan meetings with families and residents.
- Take direction from Administrator to guide actions and follow up on Administrator requests.
- Receive appropriate information from doctors and other facilities, such as History & Physicals, PASSR screenings, insurance information.
- Obtain resident clothing/personal belongings inventories, social histories, and other forms as needed.
- Collect all admission information and paperwork.
- Maintain a current list of the following and distribute to appropriate personnel:
- Authorization List (Newsletter, photo, etc.)
- Hair/Barber List
- Resident Census List
- Print out Care Plan reports for all Care Plan team members.
- Send out Care Plan Conference meeting letters and follow-up with phone calls to schedule appointments.
- Provide polite & courteous assistance to visitors, residents, salespeople, etc. in person or on the telephone.
- Complete Care Plan progress notes as needed.
- File papers in appropriate places in charts and files.
- Conduct discharge planning.
- Attend Care Plan Conferences and meet with families and staff.
- Communicate problem areas with the Administrator.
- Maintain open lines of communication with Administrator and other employees of SHC.
- Identify safety hazards and initiate immediate corrective actions.
- Assist residents as needed with such things as Medicare Part D.
- Participate in marketing.
- Is involved in community/civic health matters/projects as appropriate.
- Maintains a liaison with residents, their families, support departments, etc., to adequately plan for the resident’s needs.
- Attends and participates in staff meetings and inservices.
- Assist Administrator as needed.
