Qualifications:
High School Diploma or GED with three years experience with individuals with disabilities. Associate's Degree in a related field preferred. Relevant experience must include a minimum of one year of coordinating care, planning, creating, overseeing and implementing services, and collaborating with other service providers in a related setting. Ability to be flexible and work in a team environment. Excellent interpersonal, communication, decision-making, and problem-solving skills a must. Demonstrated computer competency required. Demonstrated ability and knowledge to work professionally with multiple service provider agencies, knowledge of Person Centered Planning, Day and Residential Habilitation. Valid NYS Driver's License required according to OPWDD/organization regulations, policies, practices and procedures.

Full time, 40 hours per week. Schedule TBD at interview.

$17.00 per hour.
