A Substance Exposed Newborn Safe Environment (SENSE) Registered Nurse provides various in-home support to Milemarkers clients. These services include; conducting home visits, completing assessments, and provide resources within the community for families engaged in DCS services. This in an in-person position requiring candidate to conduct in-home visits with infants and their parents.
Job responsibilities include:
· Conduct in-home visits within the first 30 days and last 30 days of services.
· Assess the child(ren)’s general health, including physical condition, well-being, and postnatal mood disorders.
· Assess children’s development and mother’s mental health status (Post-Partum)
· Assess the baby’s growth, and measure the head, vitals, weight, length, etc.
· Provide infant safety resources, including safe sleep practices, infant vision, feeding resources, developmental milestones, head shape, and other resources as needed.
· Coordinate with Family Connections team members, attend coordination meetings, and maintain communication.
· Attend monthly DCS SENSE Nurse calls.
· SENSE Nurse must contact the family to schedule the intake within two business days.
· The intake must be done within the first 30 days of the referral.
· Complete a SENSE Program Health Consultant Nursing Assessment report at each visit.
Requirements:
· Minimum of 21 years of age.
· Driver’s license free of any DUI’s, suspensions
· Ability to obtain a Level One Fingerprint Clearance Card
· Minimum of Associates Degree in Nursing
· Arizona nursing licensure (required)
· Preferred a Peds or NICU Nurse or a Home Visit Nurse.
· Bilingual (English/Spanish) skills preferred.
· Experience working with culturally and linguistically diverse populations.
· Ability to communicate effectively, orally and in writing, with families, social service providers, collaborative partners, co-workers, and Program Director.
· Ability to work flexible hours, including evenings and some weekends.
· Ability to work effectively as part of a team.
· Comply with all agency and program policies and procedures, including confidentiality, employee conduct, computer usage, and dress code.
· Meet program/department’s performance and productivity standards.
· Attend required agency and program/department meetings.
Pay: $30.00 - $35.00 per hour
Standard shift:
- Day shift
- Evening shift
Supplemental schedule:
- On call
Weekly schedule:
- Monday to Friday
- Weekend availability
