General Summary:

This is a full time, 40 hours per week (12 month), clinical position under the supervision of the Program Manager. The principal responsibilities of this position are to provide community psychiatric support treatment intervention to youth and families who have multiple service needs. This includes developing mutual working treatment plans and assisting clients in reaching treatment plan goals. The Youth Community Psychiatric Support Specialist will demonstrate time management skills, ability to work independently out in the community, and the ability to work in a larger team. This person is responsible for the clinical record and collaboration with other providers to provide comprehensive and client-focused care.

Principal Duties/Responsibilities:

1. Establish rapport with youth and families and assess overall treatment needs by identifying strengths that will assist the youth in reaching treatment goals. The CPS is responsible for attempting to make initial contact with any new client within 48 hours (two business days) of assignment.

2. Meet with youth and families, face to face, and provide mental health interventions that will assist clients in reaching their overall treatment goals. Face to face contact should occur at a minimum one time per month.

3. Coordinate the ongoing assessment of needs of clients and families served and monitoring systems and self-management of symptoms that might interfere with a person achieving their highest level of functioning.

4. Provide and assist in crisis intervention to client in acute distress.

5. Educate and assist youth and families in learning how to access and utilize community resources, and assist in linkage to those resources to meet the overall needs of the youth and family.

6. Meets productivity or contracted billable services per fiscal year, which is 5400 minutes per month as per their expectation. The Primary Service delivered must be within the then current agency or job specific threshold (i.e. 80%) to assure fidelity

7. Maintain equivalent documentation standards and clinical contact that matches agency and program best practice and with understanding of specific contract needs.

8. Monitor overall service delivery and advocate for youth and families with other services from other service providers.

9. Progress notes are to be completed at time of service. Achieve an annual average of at least 90% on initial score of clinical record review.

10. Educate the client and family members about mental illness, symptoms and medication monitoring.

11. Assist and empower youth and families toward self-sufficiency in accordance with the Recovery Model and development of natural social supports.

12. Ability to document best efforts to re-engage clients before termination and collaborate in cases of all transfers and referrals. Completes terminations for all clients who have been inactive for periods not to exceed requirements indicated in the Clinical Records Manual.

13. Enhance interpersonal and community coping skills, through individual and family interventions included adapting to home, school and work environments.

14. Assist youth and family in accessing all available entitlements and resources to meet the emotional and/or physical needs of the child.

15. Administers Client Satisfaction Surveys to all primary cases, annually.

16. Maintains and completes all required documentation in the Integrated Clinical record as indicated and meets all required timelines for completion of documentation.

17. Demonstrates familiarity with agency’s computer system

18. Participates in and is on time for all mandatory staff meetings, case review, treatment meetings and supervision.

19. Maintain reliable transportation.

20. Complete and maintain CPR/First Aid.

Minimum Qualifications:

An Associate’s or Bachelor’s Degree in the human services field. Experience in providing services to families and children is preferred. Access to an operational vehicle and a valid driver’s license is required. Auto insurance with 100,000/300,000 liability policy coverage must be maintained.
