Qualifications:

Education:

• Bachelor's Degree in Nursing preferred, or obtained within 2 years of hire.

Experience:

• Minimum of two years experience in case management and/or utilization management preferred, or relevant professional experience.
• Three – five years clinical experience required.

Licensure/Certification:

• Current registration in the State of NH as a Registered Nurse.

Primary Purpose:

The primary responsibility of the RN Case manager is to evaluate the medical complexity of all patients to help identify appropriate post hospital care needs. Promotes and leads efficient coordination of all aspects of patient care during the patients’ hospitalization as well as during care transitions through collaborative practice with the Healthcare Team. RN & SW Case Managers work together for a 360 view to coordinate with agencies, organizations and facilities to find the best help for their clients. Promotes patient, family, staff and physician satisfaction through coordination and continuity of care while ensuring appropriate resource utilization and maintaining high quality care. The RN case manager will provide a systematic approach to evaluation of patient care. To coordinate patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care and cost effectiveness. May need to act as a liaison with insurances and managed care representatives.
