Position Overview:
This position is responsible for welcoming members and creating a safe environment at The Gathering Place, as well as offering basic services, supporting members with navigating our different programs, and managing crises that arise. The Resource Navigator works as a part of the Case Management Program to facilitate connections to resources in the community and within the continuum of care at TGP.

Core Accountabilities:
- Works directly with program participants, on site at TGP and in the community, to provide strength-based and member-directed navigation support and housing stabilization services using culturally responsive and evidenced-based practices
- Develops relationships with program participants that are marked with dignity and respect that foster hope and empowerment
- Supports in collaborative development and execution of case management plan in partnership with program case managers and program participants
- Facilitates connection to resources and provides strategic interventions and support that promote progress as outlined by the case management plan
- Evaluates progress of program participants and adapts case management plan and/or interventions as needed to increase positive outcomes
- Provides crisis intervention as needed
- Is knowledgeable about other TGP programs and services and makes appropriate internal referrals to ensure we are offering our full range of support to meet the continuum of our members’ needs
- Makes contacts and referrals for external programs that promote stability and access to recovery supports
- Collaborates and communicates with TGP programs and external partners to ensure high quality care
- Is knowledgeable about and stays up-to-date on housing resources, public benefits, and other community resources; facilitates direct access to these resources
- Ensures completion of all program effort documentation and data entry as required, in a timely manner
- Regularly participates in supervision
- Demonstrates respect, cultural competence and knowledge of trauma informed care in interactions with TGP members, staff and collaborative partners
- Supports computer lab utilization to empower members with accessible technology

- Basic computer skills, including use of internet, email, and word processing programs
- Familiar with service programs in the Metro Denver area?
- Good customer service skills
- Able to maintain professional and appropriate boundaries
- Able to maintain confidentiality
- Able to communicate clearly and calmly?
- Able to set limits with members in a firm but respectful way?
- Able to prioritize when faced with many tasks at once
- Able to work with a diverse community?
- Able to give and receive constructive feedback?
- Bilingual Spanish and English speaker preferred
- Familiar with harm reduction and low-barrier service systems
- Relevant work experience in case management or related field preferred
- Experience working with vulnerable or marginalized populations, with an understanding of the barriers and challenges caused by homelessness and extreme poverty
- Individuals with lived experience of homelessness and/or poverty are encouraged to apply
- Bachelor’s Degree (degree in Social Work, Human Services, or a related field) or direct relevant experience preferred
