We are looking for a full-time or part-time Home Health Registered Nurse (RN) to provide skilled nursing care to older adults in the comfort of their own home. "Home" for our patients primarily is in an assisted living facility setting.
Registered nurse must demonstrate accountability and responsibility in collaborating with the interdisciplinary team to establish and achieve patient goals and maintain high-quality patient care. RNs on our team have great work-life balance since they manage their schedule and have autonomy over their care.
JOB QUALIFICATIONS:
- RN License for WI
- CPR certified
- Cover territory throughout Green Bay and surrounding areas.
- Experience in home health or preferred, but not required.
Skills, Knowledge, and Abilities: Adaptability, dependable, detail-oriented, compassionate, self-starter.
Pay: Starting $32 but negotiable based on experience.
JOB OVERVIEW: The Home Health Registered Nurse completes an initial assessment of patient to determine home care needs - complete physical assessment and history of current and previous illness(es). Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. RN will include the patient and the family in the planning process.
Assures clinical notes accurately indicate continuing communication and coordination of services with the physician, other interdisciplinary team members, and patient/family/caregiver.
Communicates with the physician regarding the patient’s needs and reports any changes in the patient’s condition; obtains physician’s orders as required.
Supervises and provides clinical direction to LPNs to ensure quality and continuity of services provided.
Participates in on-call rotation - primarily telephone triage.
