General Definition and Scope of Job:

Coordination / management of quality and compliance functions of Hospice in accordance with organizational goals, P&P, current federal, state and accreditation standards, guidelines and regulations that govern areas of responsibility. Assures that all clinical services are performed with the highest degree of quality, integrity, and operational effectiveness. Directs and performs medical record review/audits for compliance with Medicare Conditions of Participation for Hospice services as well as State and Federal Rules and Regulations. Assist in the development, implementation, maintenance of and adherence to the organization’s policies and procedures in compliance with Hospice Medicare COPs and State Regulations.

JOB STATUS: Part Time, 64 hours every two weeks

What are the Minimum Skills, Experience and Educational Requirements?

  • Bachelor’s degree in clinical patient care area preferred
  • Registered Nurses are required to be licensed to practice as a Registered Nurse in the State of Michigan.
  • The Quality/Compliance Nurse credentials include appropriate industry experience and knowledge of applicable local, state, and federal laws.
  • Preferred Experience: At least 5 years of clinical expertise. Hospice experience required
  • CHPN preferred or obtained within 1 year after hire
  • Previous management experience preferred
  • Other Requirements: Willingness to work flexible hours based on the needs of the department. * Demonstrated skill in communication both written and verbal. Skill in conflict resolution is essential.
  • Proficient use of computers.

What are the Essential Job Functions and Responsibilities?

Clinical:

  • Provides clinical care support for routine and emergent needs of the department.
  • Demonstrates adherence to infection control guidelines and ensures compliance for the protection of all staff, patients and volunteers.
  • Promotes appropriate utilization of supplies and resources.
  • Serves as a clinical resource person and assists patient care personnel in assessing patient/family status to plan patient’s care.
  • Coordination and oversight of weekly interdisciplinary team meeting.

Leadership:

  • Creates a positive work environment.
  • Demonstrates effective communication skills.
  • Effectively communicates customer service expectations to staff.
  • Demonstrates effective conflict management, coaching and mentoring skills.
  • Provides leadership and direction in accordance with organizational and agency goals and objectives.
  • Promotes open communication across the continuum. Plans and conducts unit staff meetings for each shift collaboratively with Hospice Director.

Staff Development:

  • Determines and monitors skill and competencies and ensures compliance with standards of practice required for patient care staff.
  • Collaborates with Director and Clinical Coordinator to develop and implement education for Hospice staff, patients, and SNF staff.
  • Responsible for ensuring all orientation requirements are met for new hires.
  • Responsible for ensuring all staff complete their annual education requirements.

Performance Improvement:

  • In coordination with the Hospice Director, is responsible for developing and maintaining the QAPI program for all Hospice Services.
  • Maintains current knowledge of all Medicare, Medicaid and state regulations and accreditation standards.
  • Current knowledge of NHPCO and MHHA information and activity.
  • Oversight of completion of annual regulatory/system requirements.
  • Assists Leadership to develop and implement audit/monitoring processes and systems that comply with CMS Comfortable Dying Measures
  • Responsibility for QAPI auditing and reporting
  • Identify problems and trends that may impact patient care and safety.
  • Oversees programs of process improvement
  • Develops appropriate process measures
  • Monitors the agency’s adherence to the performance improvement plan
  • Adherence to COP / LTC and other applicable regulations

  • Assumes responsibility for continually improving on own expertise.
  • Participates in continuing education and in-service programs related to achieving and/or maintaining competency in assigned skills and enhancing team functioning and effectiveness.
