Avondale House is a 501(c)(3) not-for-profit agency that provides, educational services to individuals with autism, a day habilitation program for young adults, where clients receive training in daily living skills and pre-vocational activities, employment services for those with disabilities and four residential homes for individuals unable to live in their own home. Avondale House has been serving individuals with autism since 1976.

Hours: 8:00am-4:30pm, M-F Typical, Nights/Weekends/On call Required

Classification: Full-time Regular, Salaried, Exempt

Job Duties:

  • Ensures effectiveness of individuals with intellectual development disabilities active treatment program.
  • Responsible for day to day quality assurance for the Avondale House Residential Program.
  • Develops, implements, and monitors each resident’s Individual Program Plan (IPP).
  • Coordinates, monitors, documents, and follows up on all client programs, individualized plans, and services, correcting inconsistencies and reviewing for any adjustments that may be required.
  • Develops and implements compliance policies and procedures in accordance with state and federal regulations and Avondale House policies.
  • Conducts annual and quarterly client reviews.
  • Completes ICAPS.
  • Completes E-scores for new admissions and annually for all residents to successfully rate each client during fire drills.
  • Maintains Emergency Preparedness Plan.
  • Reviews monthly data sheets.
  • Visits off-site day habilitation programs.
  • Attends IEP meetings for clients in the school and day habilitation programs.
  • Serves as liaison for other agencies and consultants.
  • Attends Health Care Management Team meeting. Communicating with the COO, Residential Manager, RN, Principal and other staff as needed to ensure the highest quality services to our residents.
  • Implementation and training of behavior programs and data collection.
  • Completes ID/RC and Living Option via SASS state website.
  • Completes critical incident data for each residential home and inquiry forms monthly via Care website.
  • Completes therapeutic passes via THMP state website.
  • Completes required Medicaid recertification forms as needed.
  • Continuously trains staff on state and federal regulations.
  • Ensures quality of program and protection of client’s rights, investigates incidents as needed.
  • Works with Residential Manager to ensure each home is in compliance with organization’s standard of cleanliness.
  • Monitors for ongoing compliance with agency directives, licensing standards, and policies and procedures.
  • Other duties as assigned.

Job Requirements:

  • Ability to demonstrate Avondale House Core Values: Commitment, Achievement, Respect, integrity, Nurturing, Growth.
  • Bachelor’s degree in Rehabilitation, Human Services, Psychology or other related field required.
  • Minimum two years of direct experience working with individuals with intellectual disabilities.
  • Expert knowledge of state and federal regulations regarding ICF and ALF regulations.
  • Experience working with individuals with autism.
  • Excellent communication and presentation skills.
  • Organization skills essential.
  • Intermediate computer skills required.
  • Current CPR certification required.
  • Must be available to work outside normal schedule as needed. You will be on-call and must be prepared to work evenings, overnights or weekends when scheduled to be on-call and as need arises.
  • Ability to manage non-compliant and aggressive behaviors according to state and Avondale House policies.
  • Ability to effectively work independently with minimal supervision.
  • Valid Driver’s License and insurability required.

Physical Requirements:

  • Fully vaccinated for COVID-19.
  • Ability to implement Non-Violent Crisis Intervention.
  • Lifting/Moving up to 50 lbs and lifting moving up to 100 plus lbs with assistance.
  • Ability to stand on your feet for 2-3 hours at a time.
  • Stooping, kneeling, pushing and pulling may be required in this position.
  • Must be capable of physically restraining clients if necessary.
  • Must be able to handle situations that may be stressful at times.
  • Must be fully vaccinated for COVID 19. Documentation Required.
