With over 100 locations throughout Southeastern Pennsylvania and the state of Delaware, Horizon House provides services to a diverse population focusing on individual strengths and choices.

Our Suburban Behavioral Health Services Region in Chester, PA is seeking dedicated Psychiatric Rehabilitation Worker. The Psychiatric Rehabilitation Worker provides Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned, facilitates group to enhance an individual’s knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing. Help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice. Monitor and track progress of individual’s in assigned caseload; participate in the development of individualized rehabilitation plans.

Provides and facilitates individual’s skill development and support which includes daily living, socialization, educational/vocational supports and opportunities, and leisure/recreation activities planning including community integration and community connections.

Participate in the resolution of crisis situations; assist with identifying individuals and resources that can provide direct support and/or immediate intervention during a crisis situation.

Establish and maintain liaisons with other service providers to promote program coordination and enhance individuals services.



Job Qualifications:


  • Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or

  • CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years
    work experience in Human Services, which must include 1 year of mental health direct service.

  • Must have excellent oral and written communication skills, as well as be detail-oriented and able to

  • complete a high volume of documentation in a fast-paced environment.

  • Strong computer skills are required, as well as Internet research abilities.


  • Ability to work independently and as part of a team.


  • Knowledge of mental health system and supports.


  • Must possess a valid driver’s license with an acceptable driving record.


  • Scope:

  • Frequent talking and listening.


  • Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.


  • Must be available for local travel.


  • May require crisis intervention for emergency situations.


  • Participation in on call coverage as applicable.


  • May be required to report to work during emergencies including inclement weather.
