Home Health Physical Therapist To Cover West Los Angeles/Santa Monica/Venice Areas

We are seeking a licensed and experienced Home Health Physical Therapist to provide skilled physical therapy services to patients in their homes. The Home Health Physical Therapist will evaluate and treat patients with a variety of conditions, including neurological, orthopedic, and cardiopulmonary disorders. Paid as an employee (W2) on a per visit rate of $75 to $90, based on experience.

Areas of coverage are throughout Los Angeles and the San Fernando Valley. Primary coverage area at this time is West Los Angeles and Santa Monica. There is opportunity for significant growth within the company as the home health service grows. Part time and full time positions available. Therapists will be paid as a W2 employee (save on your taxes). Per visit rate starts at $75 to $90/visit plus mileage reimbursement. Paid based on experience. Therapists who complete at least 25 home health visits per week will be eligible for health benefits (medical, vision, dental). Document visits with an easy to use web-based EMR that is accessible on any phone, tablet or computer. Must have a computer and internet access.

Job Description:

Provide physical therapy services to patients according to a written physician's plan of care. This may include, but will not be limited to:

● Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.

● Patient training on the use of assistive devices.

● Identify patient and family/caregiver needs for other home health services and refer out as necessary.

● Prepares and submits clinical and progress summaries based on the attainment of goals.

● Participate in discharge planning for patients.

● Prepares and submits a clinical progress summary based on the attainment of goals as directed by Organization policy.

Qualifications

● Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum

● Current California Physical Therapy license

● Current driver’s license

● Access to a motor vehicle with current and valid insurance coverage

● Current CPR Certification (Basic Life Support for Healthcare Providers)

● Verification for Employment Eligibility (US Dept. Of Homeland Security I-9 Form) if a non-citizen of the US

● Home health experience is not mandatory but a plus

Location: West Los Angeles, Santa Monica, Venice, Marina Del Rey, Culver City

Responsibilities:

• Conduct initial assessments to determine patients’ needs and develop individualized treatment plans.
• Provide skilled physical therapy services to patients in their homes, including therapeutic exercise, gait training, and manual therapy.
• Monitor and document patients’ progress, updating treatment plans as necessary.
• Educate patients and their caregivers on exercises and activities to continue between therapy sessions.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care.
• Maintain accurate and timely documentation of patient care in accordance with regulatory requirements.

Requirements:

• Licensed as a Physical Therapist in the state of California.
• Minimum of 1 year of experience as a physical therapist in a home health setting.
• Strong clinical skills and knowledge of evidence-based practice.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver’s license.
• Current CPR certification.
• Knowledge of Medicare regulations and documentation requirements.
• Ability to manage a caseload and prioritize tasks effectively.
