Job Title: Physical Therapist

Job Type: Full-time

Job Summary: We are seeking a highly skilled and compassionate Physical Therapist to join our team at a Skilled Nursing Home and Rehab facility. The ideal candidate will have excellent communication and interpersonal skills, a strong background in physical therapy, and a passion for improving the lives of our patients. As a Physical Therapist, you will be responsible for providing top-quality physical therapy services to our residents to help them achieve their rehabilitation goals.

Responsibilities: Develop and implement individualized treatment plans for each resident, taking into account their medical history, current condition, and goals for rehabilitation. Evaluate the progress of each resident regularly and adjust treatment plans as necessary to ensure maximum benefit. Communicate regularly with physicians, nurses, and other healthcare professionals to coordinate care and ensure optimal outcomes. Document all treatments, assessments, and progress notes accurately and timely in the patient's medical record. Provide education to residents and their families on exercises, activities, and precautions to promote optimal recovery and prevent further injury.

Requirements: A minimum of a Bachelor's degree in Physical Therapy from an accredited institution. Valid state license to practice as a Physical Therapist. Current BLS/CPR certification. At least 1-2 years of experience as a Physical Therapist, preferably in a skilled nursing or rehab setting. Strong knowledge of physical therapy techniques, modalities, and equipment. Excellent communication and interpersonal skills. Ability to work effectively in a team-oriented environment. Ability to maintain confidential patient information and adhere to HIPAA regulations.

Qualifications: Knowledge of physical therapy principles, practices, and techniques. Knowledge of anatomy, physiology, and kinesiology. Ability to perform evaluations and develop individualized treatment plans based on patient needs. Ability to use various physical therapy equipment and modalities. Ability to communicate effectively with patients, families, and other healthcare professionals. Ability to document accurately and timely in medical records. Ability to work effectively in a fast-paced and challenging environment. Strong organizational and time management skills.
