Under the supervision of the Director of Pharmacy, and according to standard procedure the Pharmacy Resident (Pharmacy Practice or Specialty) is a pharmacist in training who provides comprehensive pharmaceutical care through integrated drug distribution, clinical services and teaching. The resident provides services consistent with other services provided by pharmacists at UC Health. The resident training program is jointly supported by the corporate department of UC Health Pharmacy Services.
Responsibilities
PATIENT POPULATION - (CLINICAL ONLY) 5% Engages in population-appropriate communication. Has knowledge of growth and development milestones and tasks. Gives clear instructions to patients/families regarding treatment. Involves family/guardian in the assessment, initial treatment and continuing care of the patient. Identifies any physical limitations of the patient and deploys intervention when necessary. Recognizes and responds appropriately to patients/families with behavioral health problems. Interprets population-related data and plans care appropriately. Identifies and responds appropriately to different needs resulting from, unique psychological needs or those associated with religious/cultural norms. Performs treatments, administers medication or operates equipment safely. Recognizes and responds to signs/symptoms of abuse or neglect. The major job responsibilities of this position are: leadership, drug distribution, clinical intervention, teachings, training and continuing education. - 95% The following activities are components of the position to assist in meeting the major job responsibilities: Maintains competency in unique aspects of drug therapy for the patients for which they provide care, including factors related to patients of all age groups. Provides Pharmaceutical care by assisting physicians or providing direct patient care by: Makes patient care rounds focusing on drug therapy issues Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interactions. Provides pharmacokinetic consultations as required, specifically addressing drugs with narrow therapeutic indices. Concurrently monitors use of biotechnology drugs for compliance with prescribing criteria Precepts Pharm.D. clerkship students, and pharmacy residents consistent with academic credentials and clinical experience. Conducts target drug programs and medication use evaluations as needed and reports results to the Pharmacy and Therapeutics Committee Detects, monitors, documents, and reports adverse drug reactions (ADRs) and medication errors (MEs). Promotes the use of the formulary by converting non-formulary orders to formulary when possible, but coordinates procurement of non-formulary drugs when necessary. Counsels patients on admission or discharge medications as necessary Participates in the drug therapy management of medical emergencies, responds to Code emergencies and prepares medications as needed. Facilitate discharge planning and smooth transition to alternate site of care or the outpatient pharmacy. Maintains and updates a patient profile with demographics, diagnosis, allergies, and current medications. Discusses medication order clarifications with the prescriber, document any changes in patient and pharmacy records, and inform others of medication order changes. | Review the physical demands and assign an overall average percentage of time this position performs these activities. Qualifications
EDUCATION: Minimum Required: PharmD
LICENSE & CERTIFICATION: Licensed or eligible for licensure as a pharmacist in the state of Ohio. If not licensed, the candidate must be licensed by September 30 of their residency year.
REQUIRED SKILLS AND KNOWLEDGE: Communication skills, competency in dispensing pharmaceuticals, preparing IV admixtures, and providing drug information. Residents must have a Pharm.D. degree from an accredited college of pharmacy.
WORK ENVIRONMENT: While the position will work in a normal office environment, travel to the various hospitals and work locations will be necessary. Work hours will vary from time to time depending upon the needs of the business. Regular and predictable on-site attendance is required for this position.
