Summary:
The Patient Navigator helps Residents communicate with their healthcare providers so they get the information they need to make decisions about their health care. Patient Navigators are committed to removing Residents’ barriers to care by identifying critical resources for Residents, helping them navigate through health care services and systems, and promoting Resident health.
The Navigator works closely with the care team, which includes Director of Nursing, MDS Coordinator, doctors, other nurses, and other clinical staff to support positive health outcomes. Patient Navigators may also help Residents set up appointments for healthcare visits outside the facility and access financial, legal, and social support.
Essential Functions of Social Services:
- Establishes close relationships with and serves as primary point of contact for Residents as assigned.
- Schedule healthcare appointments on behalf of Residents; may accompany Residents to healthcare appointments, when required
- Deliver weekly or monthly health education or advisement to Residents in the facility
- Communicate with Care Team members (Nurses, Physicians, Aides, and other health care providers) to express Resident priorities, preferences, and concerns
- Identify resources for Residents to overcome barriers to care, such as transportation, housing, and financial challenges; connect Residents to resources to promote access to care
- May meet with Residents after primary care appointments to review and update care plan with the Care Team members
- Chart all Resident encounters, outreach, and services in electronic medical record (EMR); complete reporting requirements according to facility standards
- Attend and represent the organization at professional conferences, in-service trainings, and meetings at the request of or with the approval of supervisor
- Maintain strict confidentiality in accordance with facility policies
Qualifications of Social Services:
- Bachelor’s degree in social work or other closely related healthcare/social support field is preferred
- Short-term rehab experience is preferred
- 2 years of experience in social services background or other healthcare setting is preferred
- Candidates must maintain a working email address and phone number for employer communication.
- Ability to work cooperatively as a member of a team
- Ability to communicate effectively with residents and their family members, and at all levels of the organization
- Ability to react decisively and quickly in emergency situations
- Ability to maintain confidentiality
- Ability to read, write and speak the English language
- Knowledge of training techniques for clinical staff
- Good Ethical Practice
- Relationship management
- Basic computer knowledge
- Strong written and verbal skills
