Job description
This full-time position will provide direct services and conduct activities to increase access to healthcare and other community resources with the goal of promoting positive health outcomes in the Sexual Health and Wellness Center at Columbus Public Health (CPH). Additionally, it will conduct outreach to support and promote sexual healthcare services with linkage to CPH services and community partners as appropriate.
Preferred experience and training:

Background in reproductive health, sexual health, or public health;

Knowledge of community resources;

Community health worker certification;

Bilingual in Spanish.
Urges target citizens to seek professional help and guidance from programs and services;
Follows up on contacts and submits reports of outreach activities;
Seeks out individuals throughout the community and speaks with them to become familiar with their problems;
Participates in community and neighborhood events and activities calculated to acquaint the residents of an area with services or programs;
Speaks before community groups to establish rapport between community and program representatives and to publicize available programs and services;
Arranges transportation for individuals to testing and treatment facilities or other community-based services;
Contacts other agencies and groups to solicit their aid and cooperation with outreach programs;
Learns to maintain records on individuals who require treatment and/or participate in the programs;
May prepare news releases and promotional material concerning outreach programs and services;
May need to communicate with individuals who have limited English language skills;
May provide child care services that serve to enable clients to receive testing or treatment;
May perform on-site visits to businesses to review compliance status and submit written reports on findings;
May be assigned to serve as a member of an Incident Command System (ICS) Team , or a similar public health response team which may include the conduct of operations on a 24/7 basis at remote locations. Assignment may include serving as a department and/or community incident commander or on the incident command team.
Possession of a valid driver’s license.
Some positions may require specialized skills, background, and/or certification due to the nature of, or funding requirements for, the program.
