Job description
Must be fully vaccinated for COVID-19 including two doses of a 2-dose series or a single dose of a 1-dose series prior to start date. Fully vaccinated is defined as 14 days post series completion date.

The ASC Nurse Navigator is responsible for assisting with the coordination of care for patients of the Vancouver Clinic Ambulatory Surgery Center (ASC). They will serve as a consistent link between patients and all other caregivers, with a high focus on the being properly prepared for the day of surgery and closely monitored post-operatively. Will collaborate with physicians and healthcare providers to ensure the highest quality of care and utmost patient satisfaction. The ASC Nurse Navigator provides continuity of care coordination thereby providing a seamless, coordinated process that supports the patient’s need for education, care coordination and psychosocial support across the surgical experience. The ASC Nurse Navigator will also serve as a visible and available resource to nursing staff regarding education and patient-specific opportunities. The ASC Nurse Navigator will monitor the provision of quality and efficient healthcare services and resources to promote optimal clinical, financial, and patient satisfaction outcomes across the continuum of care.
SCHEDULE : 4 X 10 - HOUR SHIFTS, 7AM TO 5:30PM. ON SITE
ESSENTIAL FUNCTIONS and RESPONSIBILITIES:
  • Performs care coordination. Works in conjunction with multiple physician offices to coordinate additional exams, procedures/treatments and pre and post-op care; involves appropriate family members in teaching; utilizes available education resources.
  • Performs nursing assessments and triage of patient care needs for new and ongoing patients
  • Oversees the planning, scheduling, and implementation of day-to-day clinical activities and procedures for navigated patients.
  • Central point of contact for all navigated patients; including interaction with medical, nursing, and ancillary services.
  • Provides education to patients, families, and significant others; acts as an information resource to other health care professionals.
  • Extensively and accurately documents information in the Electronic Medical Record (EMR).
  • Effectively communicates.
  • Establishes and maintains a plan of care across a continuum. Collaborates with the patient, family and health team members including interventions, goals, outcomes and education to facilitate desired patient outcomes.
  • Implements nursing and medical interventions using appropriate resources, skills and knowledge; assists in perioperative care.
  • Takes initiative to coordinate accreditation requirements through national organizations (AAAHC, CMS, etc).
  • Collect and report pertinent clinical data to monitor patient outcomes.
  • May participate in the orientation, training and evaluation of new staff.
  • Follows established departmental policies, procedures, and objectives and all safety, environmental and infection control standards.
  • Designs or participates in the creation and implementation of health education materials for new or existing service lines for patients and staff.
  • Performs miscellaneous job-related duties as assigned.
Skills and Abilities:
  • Demonstrated knowledge of perioperative nursing in the outpatient surgery environment.
  • Basic knowledge of coding and referral process for outpatient surgery.
  • Demonstrate ability to work independently with minimal direction and lead projects with confidence.
  • Proficient in required software products (EPIC, OpTime, Excel).
  • Demonstrates strong organizational skills and can manage several projects with various teams of people.
  • Ability to communicate articulately and professionally present to all levels of the organization.
  • Must be able to function efficiently in a high performing, results oriented work environment.
EDUCATION AND EXPERIENCE:
  • Graduate of an accredited school of nursing.
  • Bachelor’s degree in nursing strongly preferred
  • Current unencumbered WA State RN licensure required.
  • Current American Heart Association Health Care Provider BLS CPR certification
  • Minimum of three (3) years’ experience as a registered nurse.
  • Ambulatory/Outpatient Surgery experience preferred.
  • Experience in a LEAN healthcare environment with application of PDCA preferred
Pay Range:
$41.77 - $62.65

PTO and Personal Time accruals are pro-rated by FTE/hours worked. Non-benefits eligible employees will accrue Personal Time based on hours worked and State worked.
