Position Summary:

The Medical Records Coordinator ensures that information regarding care in the Hearthstone Health Center is presented in a manner consistent with established health information policies and procedures.

Duties/Responsibilities:

  • Obtain and supervise the admission information.
  • Transfers of resident medical information to/from Health Center or to a different level of care within the community.
  • Inputs resident clinical and demographic information into the computer system.
  • Maintain Admission Number Register, Census Register, Master Patient Index in the Computer, Emergency Card File and Death Register.
  • Keep a daily tally of all residents in the computer census program.
  • Code resident’s diagnosis according to current ICD 10 coding system in collaboration with Nursing and Therapy Teams.
  • Monitor all records to ensure proper compliance with Federal & State regulations.
  • Monitor each resident’s chart for current information and timely updates.
  • File all completed records according to present system used, maintain archives.
  • Audit charts for completeness after admission: 72 hrs., 7 days 14 days, 21 days, quarterly and prn.
  • Coordinates the Triple check process and drives communications with the Billing and Accounting teams for skilled stays.
  • Attends and participates in Medicare meetings.
  • Initiate Medicare and/or commercial insurance forms for all residents being reviewed in Medicare/Skilled meetings and as necessary.
  • Type and deliver appropriate letters for Social Services.
  • Attend all workshops and educational trainings that pertain to medical record duties.
  • Perform related duties and generate reports, as requested by the D.N.S.
  • Keep current on all regulatory changes.
  • Complies with insurance company requests for information on residents’ status.
  • Completes auditing functions on charts for Health Center, and Assisted Living/Memory Care residents.
  • Assemble and breakdown charts and maintain back-up charts for Health Center and AL residents when discharged from different levels of care.
  • Manages Resident health information according to HIPAA guidelines.
