TITLE: MDS Coordinator RESPONSIBLE To: Director of Nursing

JOB OBJECTIVE: Schedule and conduct patient care reviews on each resident in keeping with facility policies and programs, and consistent with licensing, certification, and other regulatory agency rules and regulations. This responsibility is assigned to a registered nurse in the role of Assistant Director of Nursing, Unit Coordinator, or Shift Nursing Supervisor.

JOB QUALIFICATIONS/DUTIES:

  • Covid-19 Vaccination Required
  • Graduation from an accredited school of nursing.
  • Current Registered nurse license in good standing with State Board of Nursing in the state of North Carolina.
  • Previous supervisory experience as a charge nurse or in a related supervisory role.
  • Previous experience as a licensed nurse providing direct care to the aged or chronically ill, developmentally disabled or mentally disordered in a long term care facility.


Job Responsibilities and Duties:

  • Maintains needed records that indicate when periodic patient care reviews are due.
  • Schedules residents to be reviewed during weekly care review session, and posts schedule in advance for staff, residents, and family information.1
  • Meets with the facility Director of Social Services to plan scheduling so the resident, his/her legal representative, and family members can be provided an advance notice of the meeting date, and invited to attend.
  • Responsible for coordinating all disciplines and resource personnel for the most effective strategy to formulate Resident Care Plans.
  • Chairs the patient care review meeting, keeps before the participants the purpose and goals of the review, and facilities the appropriate conclusion of the review.
  • The assessment coordinator is responsible for obtaining appropriate signatures on MDS quarterly review and care plans, prior to or immediately after the care plan session.
  • Patient Care Coordinator is responsible for the quarterly and annual review of the MDS on each resident.

Schedule:

  • Hybrid ( 3 days in the facility and 2 days work from home)
  • Monday-Friday 8am-5pm
