Centerpath Wellness, a division of Easterseals New Jersey, is seeking a Nurse Case manager to assist with an array of clinical services for our clients and their families in our Plainfield New Jersey office!
- Your responsibilities also include supervising Injection Nurse and conducting psycho-educational groups with clients and significant others as needed.
- You will participate in clinical team, staff, and committee meetings and training.
- Effectively manage a caseload of clients providing treatment and discharge planning
- Provide a variety of counseling and education to clients and/or significant others through health and nutritional counseling, medication management, and other psychoeducational information
- Ensure the accurate and timely completion of the documentation in the Electronic Health Record (EHR) system
- Communicate with and maintain relationships with drug representatives, physicians, and insurance companies to secure successful client outcomes and submission of prior med authorizations
- Assist prescriber with the interpretation of lab work and maintenance of medication logs
- providing injections to clients who receive medication by injection.
- Assist in the refill of non-controlled substance medication as approved by APN.
- Uphold the agency's Code of Ethics and Client Rights Policies and Procedures
- Maintain compliance with all regulatory policies and procedures of Center Path Wellness, as well as all regulatory bodies including but not limited to: DMHAS, DHS, Federal, State, and County Governance, Medicaid, and Medicare.
- Respond in a timely manner to internal and/or external phone calls, voicemail messages, etc. to promote service delivery and communication
- New Jersey Licensed Practical Nurse (LPN) and two years working with the psychiatric population, or;
- Licensed as a Registered Nurse (RN) in the State of New Jersey with a minimum of 1 year post-licensure experience with the psychiatric population.
- Comfortable with using computer technology; knowledge of EHR is helpful.
