We are currently seeking qualified candidates for the role of Licensed Practical Nurse (LPN).

Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team in an assisted living facility. The LPN will be responsible for providing nursing care to our residents in accordance with physician orders and established policies and procedures. The LPN will also be responsible for supervising and providing guidance to nursing assistants.


Duties and Responsibilities:

    • Administer medications and treatments as prescribed by physicians
    • Monitor and document changes in residents' health status
    • Assist in the development of care plans and ensure their implementation
    • Provide direct nursing care to residents and assist them with activities of daily living
    • Observe and report any changes in residents' condition to the physician and the Director of Nursing
    • Respond to emergencies and provide appropriate interventions
    • Communicate effectively with physicians, residents, families, and staff members
    • Participate in the orientation and training of new staff members
    • Ensure that all nursing care is provided in compliance with federal, state, and local regulations
  • Participating in the review of clinical indicator reviews falls, wounds, nutrition, antipsychotic use, etc.
  • Other duties as assigned by Director of Health & Wellness/Administrator
    • Current PA state licensure as a Licensed Practical Nurse (LPN)
    • Minimum of one year of nursing experience in a long-term care or assisted living facility
    • Knowledge of nursing principles and practices
    • Ability to work collaboratively with other healthcare professionals
    • Excellent communication and interpersonal skills
    • Ability to prioritize tasks and manage time effectively
    • Basic computer skills


Physical Demands:

The LPN may be required to stand and walk for extended periods, lift and move residents, and perform other physical tasks as necessary. The LPN must be able to perform all essential job functions with or without reasonable accommodation.


Work Environment:

The LPN will work in a fast-paced and challenging environment that requires flexibility and adaptability. The LPN will work closely with residents, families, and other healthcare professionals. The LPN may be exposed to infectious diseases and must follow established infection control procedures.
