SUMMARY The Intake Coordinator is the initial agency contact for referral sources seeking Transitional Living Services of Northern New York services on behalf of consumers. The coordinator secures information including medical, psychiatric, and social factors contributing to a client’s request for services. The coordinator evaluates this information and the applicant’s capacities and abilities. Face to face screening appointments are conducted with applicants to assure proper services are matched with recipients prior to admission. The coordinator makes admission recommendations to the Program Directors. The intake coordinator is also responsible for compiling records and preparing statistical reports on referrals and admissions. This position requires extensive collaboration with other community resources including attendance at Single Point of Access (SPOA/E) meetings in Jefferson, St. Lawrence Counties and Lewis Counties.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Receives all referrals for services and reviews referral packet for completeness
Obtains all available and necessary paperwork for the referral
Schedules and conducts screening and intakes
Makes necessary arrangements for admission to programs
Ensures all available funding sources have been accessed to maximize the referral’s ability to pay for service, if appropriate
Records referral and screening information as required
Arranges pre-placement tours
Assists in the development of the Initial Action Plan by ascertaining during the screening process what areas the referral needs assistance with
Maintains statistics on referrals
Attends SPOA/E meetings in Jefferson, St. Lawrence and Lewis Counties.
Plan and conduct outreach activities to maintain flow of referrals from current and new referral sources.
Work with funding sources to establish a working relationship
Maintains contact with the potential participant and the referring source while on the waiting list,
to ensure continued interest in and eligibility for services
Include other stakeholders in the community in the planning and delivery of services for referred adults and children
Develop a process to manage slot vacancies and place the highest needs individuals in these slots as they become available
Chair admissions committees as scheduled
Other related duties as assigned.

Experience and Skills:
EDUCATION and/or EXPERIENCE
Bachelor’s degree in the Human Services field; and two years related experience and/or training working in the mental health field, particularly with developmentally delayed, geriatric and co-occurring disorder clients. QMHS required.
