JOB SUMMARY:
The Intake Coordinator is responsible for the coordination and the processing of all incoming homecare referrals for our Ohio homecare offices. This professional is also responsible for obtaining all required information and documentation to process the referral in a timely/ accurate manner, as well as all data entry into the computer system.
OUALIFICATIONS:
- High School Diploma.
- Minimum of one (1) year experience in Intake preferred.
- Acceptance of philosophy and goals of this Agency with the ability to exercise initiative and independent judgment.
REQUIREMENTS:
- Current knowledge of State and Federal homecare regulations.
- Customer service and phone etiquette skills
- Strong written and verbal communications
- Able to work independently
- Basic understanding of Insurance verification process
- Previous experience with use of EMR systems/superior computer skills.
- Demonstrate ability to maintain composure under stressful conditions
RESPONSIBILITIES:
- Understands and adheres to established policies and procedures.
- Implements the Intake process for each patient.
- Assists with initiation of skilled services. Notifies staff via email of pending admissions and schedules Admission visits
- Attends daily stand-up and reports on admissions status, hospitalized patients, Resumptions or other pt related matters as appropriate.
- Prepares/uploads clinical notes, SOC orders and Face to Face documents to clinical record. Fields calls pertaining to referrals and screens for appropriateness.
- Processes all appropriate referrals via email, fax, Allscripts, and other various Platforms, and communicates with the Regional Director, Branch Manager and Office Manager.
- Verifies demographics, contact information, primary care physician, and Insurance coverage for all new and returning patients.
- Enters referral info into EMR system to establish new admission including uploading required clinical information and orders.
- Communicates with the patient/family, clinicians and other team members to coordinate the initiation of services.
