Assist clients in locating suitable housing in the community
*
Complete person-centered housing plan
*
Assist client in locating and applying for public/community benefits resources
*
Performs outreach to the community and other appropriate agencies regarding Housing
*
Communicates regularly with clients and their support teams to discuss progress, problems, and plans
*
Take clients to tour potential housing sites
*
Assist clients in completing housing applications
*
Assist with packing and move coordination
*
Follow up on a regular basis once move is completed to assist in housing stability (sustaining)
*
Maintain progress notes as required
*
Attend weekly team meeting and one on one meetings as required

Pay: From $18.00 per hour
