SUMMARY:
The role of a hospice social worker is to advocate for each patient’s end-of-life wishes and help individuals address the emotional aspects of late-stage illnesses. Hospice social workers also assist families in identifying other available local services and resources for additional support. They are to assess the status and needs of hospice patients and families and be available to provide counseling, crisis intervention and support to hospice patients and families.
Duties and Responsibilities:
Provide medical social services in accordance with the POC
Participating in the developing and revising of the POC
Assess emotional factors related to terminal illness
Assess the patient and family for risk factors related to suicide and or abuse and neglect.
Provide crisis intervention
Provide information pertaining to preparation of advance directives, funeral planning issues and transferring responsibilities regarding fiscal, legal and health care decisions.
Address patient/family questions and issues
Assess bereavement needs
Communicate effectively, respectively an d professionally with patients and families.
EDUCATION, TRAINING, AND EXPERIENCE:
One year experience in a health care setting
Has a MSW degree from a school of social work accredited by the Council on Social Work Education or:
Has a Baccalaureate degree in social work from an institution accredited by the council of Social Work Education, or a baccalaureate degree in psychology, sociology or other field related to social work and is supervised by an MSW
REQUIREMENTS/QUALIFICATIONS:
Must be able to read, write and speak English
Possess good written and verbal skills
Must meet deadlines and work under limited supervision
Demonstrated ability with Office software
Must be able to climb stairs and stand and walk for a period of at least 4 hours
Possess a valid drivers license
Proof of motor vehicle insurance
